RN – Care Coordination – Home-Based Primary Care team – 20 S Park at University of Wisconsin Medical Foundation in Madison, Wisconsin

We are seeking an RN – Care Coordinator to:


  • Be an integral part of the interdisciplinary team of professionals providing holistic care coordination to our homebound patients.

  • Ensure high-level, integrated, and personalized care for older adult patients with complex medical and psychosocial needs.

  • Improve health outcomes by improving communication among health care and community providers.

  • Be the future of patient care.

Education:

Minimum – Graduate of school of nursing

Preferred – Bachelor of Science in Nursing (BSN) or higher

Work Experience:

Minimum – One (1) year experience as registered nurse

Preferred –


  • Previous experience in adult acute care or ambulatory setting

  • Previous Case management experience.

Licenses and Certifications:

Minimum – Registration as a registered nurse in state where employed or licensure in the state licensure compact. CPR/BLS.

Preferred – Case Management Certification.

Required Skills, Knowledge, and Abilities:


  • Excellent communication, problem-solving, conflict resolution and interviewing skills.

  • Excellent prioritization and organizational skills.

  • Ability to work as a team member and as an independent service provider.

  • Excellent verbal and written communication skills, including grammar and spelling.

  • Knowledge and experience documenting in an electronic medical record

  • Ability to educate staff

  • Ability to provide counseling and emotional support with compassion and respect for the individual

  • Ability to provide support to members of the care team

  • Ability to maintain confidentiality

Physical Requirements:

Light: Ability to lift up to 20 pounds maximum with frequent lifting and/or carrying of objects weighing up to 10 pounds.

Work Schedule:

This is a full-time, 100% FTE position scheduled to work 40 hours per week. The scheduled shifts are Monday to Friday, 8:00 am to 5:00 pm. Hours may vary based on the needs of the department.
