DaVita is looking for a Full-Time Nurse Practitioner (NP) Manager to join our Integrated Care team.

The Nurse Practitioner Manager, "NP Manager", leads and manages our Integrated Kidney Care Nurse Practitioners in a designated market or geographical region. This Nurse Practitioner Manager role will be located in Minneapolis, MN and will lead a team of strong Nurse Practitioners in our Minneapolis, MN-Green Bay WI market.

This Nurse Practitioner position is great for experienced Nurse Practitioners who want to continue to develop and grow their careers, as well as continue to grow and develop other clinical professionals.

In this role you will provide clinical leadership, oversight, coaching, supervision and onboarding of Nurse Practitioners to ensure that quality, cost-effective care is provided for CKD and/or ESRD patients.

You will participate in population health management and overall financial viability of the program. This role functions autonomously, yet in partnership with operational and clinical leadership on budgeting, quality initiatives, outcomes and performance measurements.

ESSENTIAL DUTIES & RESPONSIBILITIES

  • Serve as the clinical lead for a team of NPs, and oversee their work. Ensure NP CHE quality and productivity, including documenting findings, appropriate diagnoses, closing Plan of Care gaps, and accurate coding. Support NP hiring efforts and oversee onboarding of new NPs with the training team, assessing competency gaps to close. Support ongoing training for NPs through clinical best practice exercises (At The Elbow and Card flipping); provide ongoing support and oversight. Provide ongoing clinical guidance and training to NPs to improve understanding of CKD, ESRD, and commonly associated chronic conditions.
  • Serve as key leader of IKC clinical and operational strategy working with IKC Medical Officer and Ops leadership. Focus on care delivery and interventions to support effective and appropriate medical cost management and focus on preventing/reducing hospital re-admissions through proactive post-discharge planning and follow-up with patients most at risk, and non-CVC outpatient starts for CKD members. Identify opportunities to streamline NP workflow and focus management intensity on high risk, high-utilization members.
  • Lead complex case reviews and timely support of complex case escalations of high utilization and high-risk members working with a team of specialized resources and patient's care team, and payor resources. Advise on closing POC gaps with care team to effectively and swiftly identify the root cause and interventions to reduce hospitalizations and deliver on vision of best in class integrated care and patient quality of life.
  • Know and comply with DaVita policies and procedures in accordance with State and Federal regulations for patient care and services (CMS, HIPAA, OSHA, or other governing agencies); Standards necessary to maintain organizational accreditation (such as NCQA); and, State or Federal regulations of Injury Prevention and Safety Training Program
  • Other duties as assigned.

Preferred Experience:

  • 4-6 years of NP Experience; experience in population health, Primary/family care, and/or nephrology a plus.
  • Masters degree in Nursing.
  • At least 1 year experience in supervision or leadership (did not have to be a NP at time of supervision or leadership)
  • Excellent verbal and written communication skills
  • Demonstrated proficiency in clinical assessment, diagnosis, planning, implementation, documentation, and evaluation regarding the needs and care of chronic disease
  • Deals with confidential information and/or issues using discretion and judgment
  • Intermediate computer skills and proficiency in MS Word, Excel, and Outlook
  • Excellent organizational skills and attention to detail.
