Job Description:

We are a 24 hours facility with three (3) shifts and the vacancies available at the time of hire are not known when the position is posted.
 
External Intermittent appointments are used to support and supplement a regularly scheduled workforce. The position has LIMITED benefits. This is NOT a full-time position. Regular hours of duty in a two-week period are NOT guaranteed or consistent. Intermittent status employees are encouraged to apply for permanent vacancies. 

This position requires a high school diploma or equivalent (i.e. GED). If your education is outside the United States you will be required to submit an education evaluation showing how your education equates to the US standard. 

DUTIES:  In the work area assigned trains individuals to function independently in self-help &/or daily living skills (e.g., personal hygiene, eating assistance, grooming, etiquette, social skills, basic housekeeping) by using a variety of training techniques (i.e., modeling, verbal prompting, physical prompting, environmental modification, trauma informed care approach) according to the agreed upon active treatment plan developed by the Interdisciplinary Team; provides crisis intervention for unsafe behavior as defined by active treatment plan (Person Centered Plan (PCP)), plans leisure time activities (e.g., games, crafts, walks) to structure free time & assist with related scheduled activities (e.g., operates motor vehicle to transport individuals to recreational, educational &/or occupational activities: swimming, games, learning activities, etc.).  Utilize appropriate intervention techniques to train & maintain a supportive & positive learning environment (e.g., uses positive reinforcement, encouragement & prompting, an atmosphere of choice & independence to the extent safely possible); in conflict or threatening situation, uses approved intervention techniques to deescalate conflict or prevent harm; accompanies & trains individuals in programs on & off campus as described in the individual’s PCP; performs other duties providing personal care & maintains assigned supervision levels. cooperates & participates as a part of the team; shares relevant information & observations in writing, as well as verbally; briefs incoming staff on critical incidents of the shift.

Participates directly or indirectly in development of individual’s goal-oriented active treatment plans in cooperation with professional & para-professional staff by attending team &  staff meetings, participates in comprehensive evaluation meetings, assists in writing PCP & recommends changes in PCP as observations documentation of progression/regression indicate; maintains confidentiality & completes charting/documentation, reports (e.g., individual behavior & progression/regression in relation to active treatment plan); delivery of services to individuals; reports reactions to medications or any other unusual incidents; changes or responds to needed changes in the active treatment program for individual; shops for & with individuals for clothing &/or personal belongings. Ensures individuals are dressed appropriately; ensures that Trauma Informed Care approach is used daily to carry out & respond to responses affecting treatment plans for individuals. Acts as a mentor & assists with training duties for new staff.

Provides customer service (e.g., greets visitors, answer questions, provides feedback, renders direct care, etc.) to families, co-workers & residents, ensure professional & courteous interactions with internal & external customers.  

Operates equipment necessary to train individuals to be more independent as described in the PCP (e.g., adaptive equipment, washer, dryer, dishwasher, vacuum, kitchen appliances, paper shredder); performs housekeeping such as taking out trash & dirty linens, wiping down tables, beds, walkers, wheelchairs etc., mopping, dusting and laundry as needed by assisting in day to day upkeep of a clean & sanitary living environment.  Assist & train individuals how to mop floors, wash windows, dust furniture, complete personal laundry, & empty trash; utilizes adaptive equipment (e.g., pushes wheelchair, assists with walkers, operates lifts, uses gait belts etc.) & maintains adaptive equipment/devices (e.g., wheelchair, mechanical lifts, walker, orthotics, hearing aid, plates/cups/silverware etc.); works with new employees; participates in group projects as requested; assists nursing (e.g., applies ointments/lotions); completes other related duties as assigned.

Attends in-service training &/or educational programs; maintains annual training; other duties as assigned to ensure the safety and success of the individuals supported & ensure daily operations of the center.

This position includes mandatory, additional hours &/or overtime.

Formal education in arithmetic that includes addition & subtraction & in reading, writing & speaking common English vocabulary. In Department of Developmental Disabilities, must also possess a high school diploma or a certificate of high school equivalence pursuant to Chapter 5123:2-3-01(F) (1) (c).

For positions that require operation of motor vehicle to transport consumers to recreational, educational &/or occupational activities, a valid driver’s license is required.
