ABOUT THE JOB
Sally and Anthony Mann Center is a Residential Treatment Facility that serves emotionally vulnerable girls ages 12-21, who are challenged by trauma-related and psychiatric issues. The RTFs provide supervised living units and programs of therapeutic and educational services that emphasize family interaction and strengthening of community support networks.
Sally and Anthony Mann Center is currently seeking a Transition Therapist to work as part of a treatment team to ensure that youth and families drive the focus of their treatment and are full-time partners in their own healing and recovery. This is a full-time, unionized role in Hawthorne, NY.
Responsibilities include, but are not limited to:
Utilize assessments to inform clinical practice and treatment planning
Initiate and maintain contact with collateral service providers throughout admission and post-discharge
Participate in program development through attendance at meetings and committees
Prepare court reports and relevant legal documentation and attend court as clinically indicated
Identify, establish, strengthen community linkages and resources
Be responsible for development and implementation of comprehensive discharge plan
Support the integration of coping skills with youth and family in the home and community
Utilize youth and family strengths and interests to enhance prosocial community engagement and development of natural supports
Provide comprehensive aftercare services to discharged youth and their families in their home communities
QUALIFICATIONS
The Jewish Board is currently looking for a talented Transition Therapist of all cultures, religions, races, and gender expressions with the following qualifications:
A Licensed Master of Social Work (LMSW) or Licensed Clinical Social Worker (LCSW) is required
A minimum of one (1) year of relevant experience working with a similar population
A current, valid clean Driver's license is required for this role
