The Clinical Pharmacist, Care Programs will provide support to CDPHP's clinical pharmacy programs including but not limited to behavioral health pharmacy programs, transitions of care/Hospital Experience program and the drug utilization review (DUR) program. The Clinical Pharmacist, Care Programs will work as part of a collaborative care team focused on transition of care from acute care settings to outpatient community providers. Responsibilities include completing comprehensive medication reconciliation and review, with resultant patient education services, for identified members that focus on optimizing medication use, improving health outcomes, and preventing avoidable medication-related ER/hospital visits. By working closely with CDPHP behavioral health and medical care managers, the Pharmacist will assist in identifying medication access issues and social determinants of health that may contribute to improper medication use. This role will also participate in drug utilization review program activities across all lines of business.

QUALIFICATIONS:


  • Advanced degree required.

  • Doctor of Pharmacy (PharmD) or equivalent degree from an accredited pharmacy school required.

  • Graduate of an ASHP accredited PGY-1 residency OR board certification in an applicable area of practice is preferred.

  • Eligibility for licensure in New York State is required and registration must be renewed and maintained throughout employment.

  • A minimum two (2) years of experience in a Clinical Pharmacist/Managed Care pharmacist role required.

  • A minimum two (2) years of experience in a clinical role that involved interaction with physicians, patients, and other professionals for medication therapy consultation required.

  • Transition of care medication reconciliation and comprehensive medication management service experience preferred.

  • Knowledge of claims/pharmacy systems preferred.

  • Experience with Microsoft Office, including Word, Excel, Outlook and PowerPoint is required.

  • Demonstrated ability to work collaboratively with multiple partners to positively affect outcomes at the team, organization and member level.

  • Must be detail-oriented with strong organizational skills, including the ability to coordinate workflow.

  • Demonstrated ability to pro-actively identify problems, as well as recommend and/or implement effective solutions.

  • Demonstrated ability to provide excellent customer service and develop relationships both internally and externally.

  • Demonstrated ability to work with and maintain confidential information.

  • Excellent verbal and written communication skills.

  • Flexibility to adapt to a changing and fast-paced environment.

  • Ability to participate in on-call rotation, which may include holiday rotation as scheduled.

  • Ability to travel locally, including access to reliable transportation. If driving vehicle for CDPHP business, employee must have valid driver's license, current inspection and appropriate insurance coverage in effect at all times.
