PURPOSE OF THIS POSITION:
The Community Connector is responsible for the correct implementation of periodic services on their team. The position is primarily for empowering the individuals supported to build and maintain community connections and natural supports while also building professional relationships between our agency and the community at large.

ROLE AND RESPONSIBILITIES
• Ensure NC licensure standards and Medicaid guidelines are followed.
• Coordinate periodic services with the person supported and their support team, identifying time, place, and activity. This schedule will be flexible to accommodate the activities the person receiving services chooses.
• Function as a Direct Support Professional (DSP), for the provision of periodic community-based services.
• Support several individuals in a case load format.
• Responsible for assisting the persons supported to find activities of interest to participate in throughout the week, to fully utilize services they are authorized to receive.
• Maximize hours worked as billable services and document service breaks with Service Coordinator as well as through service notes.
• Build and maintain professional relationships with other providers, companies, volunteer organizations, etc. to provide the best community supports possible.
• This position is largely self-directed while out in the community, therefore it is held to a high expectation of professionalism and therapeutic, person-centered support.

OTHER POSITION REQUIREMENTS
• Ensure the promotion and protection of individual human rights; ensure confidentiality of any information related to persons receiving supports; ensure that individuals keep and maintain personal possessions; ensure that persons are free from abuse, neglect, exploitation, or punishment.
• Provide encouragement to individuals to achieve skills or behaviors in areas such as independent living, socialization, meal preparation, budgeting, money management, recreation and leisure activities.
• Monitor the overall health and safety of the individuals served including providing information to medical professionals (psychiatrists, general practitioners, specialists, etc.). Assuring that issues and concerns about health and safety are brought to the attention of team members when they arise; and that staff are communicated with regarding treatments and interventions.
• Attend team meetings and staff training as requested.
• Communicate effectively and follow instructions, both oral and written.
• Maintain program standards as required by regulatory agencies.
• Use a person-centered approach to providing services, and demonstrate effective interactions, training techniques, and interventions for other support team members.
• Turn in billing and all other required paperwork on time.
• Maintain periodic schedules and ensure shifts coverage when team members call out or request time off.
• Accuracy is essential in all aspects of this work. Accuracy and timeliness in record keeping is critical to the agency adhering to state, licensure, rules and to avoid penalties or paybacks.
• Errors in judgment or delay of efforts could result in consequences with licensure and regulations.

QUALIFICATIONS AND EDUCATION REQUIREMENTS
The required minimum qualifications for this position include:
• High school diploma/GED
• Highly motivated self-starter
• Expert level organization and professionalism
• 3+ years full-time experience in a related human service field
• Valid NC driver’s license, acceptable driving record and appropriate automobile insurance
• Satisfactory background check and Healthcare Personnel Registry report
• Proficient and confident with applicable software, hardware, and technology
• Competency in the following areas: technical knowledge, cultural competency and awareness, analytical skills, decision-making, interpersonal skills, communication skills, clinical skills, person-centered practices, evaluation and observation, crisis prevention and intervention, professionalism and ethics, community living skills and supports, health and wellness, community inclusion and networking, empowerment and advocacy, safety, education, training, and self-development

This position requires the staff member to be able at any time to physically perform the following tasks:
