Reporting to the Clinical Manager, he or she will act as a resource for the intake staff and scheduler to insure the smooth functioning of the intake, acceptance and scheduling process. This includes responding to triage of patient telephone calls as well as being an information conduit between clinical leaders, clinicians and physicians and or physician's representatives.
Bachelors or Master's Degree in Nursing or the equivalent experience
Minimum 1-year experience, preferably in a home care setting
Leadership experience, collaboration in a multidiscipline environment
Licensure and/or Certification
Current license to practice as a Registered Professional Nurse in MA.
Current knowledge of home care nursing practices
Ability to respond to rapid changes in patient scheduling.
Must be able to work in a collaborative team environment
Must be able to communicate with clarity and provide direct feedback
Ability to work effectively as team member
Excellent customer service skills relating to physicians, clinicians, patients and families
Excellent organizational and prioritizing skills.
Computer literacy in Microsoft Word, Excel and Outlook.
