TheState of ConnecticutDepartment of Developmental Services (DDS) is a state agency which provides services to individuals with developmental disabilities and their Families. We are seeking individuals who are looking to advance their career and make a difference!


We are currently recruiting highly motivated Developmental Services Case Managers for existing and anticipated vacancies. Work locations will be in offices located in the following regions:



  • North Region: East Hartford, Hartford, Newington, Putnam, Windham (Willimantic)

  • South Region: New Haven, Norwich, Wallingford

  • West Region: Cheshire, Danbury, Norwalk, Stratford, Torrington, Waterbury

  • Southbury Training School: Southbury


Applicants selected for hire should expect to be assigned to one of the above listed worksites.


These are full-time thirty-five (35) hour per week positions. The work schedule is first shift, Monday through Friday. However, must be flexible in order to meet the needs of the agency and individuals and families we support.


NOTE: These are permanent full-time positions. However, candidates may be initially hired into temporary schedules and temporary work locations. Per a stipulated agreement, employees hired into temporary schedules and temporary locations will be assigned permanent functional work assignments at a future date. These permanent work assignments may be at different locations with possible slight schedule variations. Future schedules will still be full-time Monday-Friday on 1st shift.


Current DDS Case Managers should NOT apply.

PURPOSE OF JOB CLASS (NATURE OF WORK)



In the Department of Developmental Services and the Department of Social Services this class is accountable for independently performing a full range of tasks in providing case management services forindividuals with intellectual disability and their families to ensure the delivery of appropriate medical, educational/vocational, social, residential and other services and conformance withfederalregulations when specifically assigned.



EXAMPLES OF DUTIES



Performs activities related to individualservices plans for conformity with federal and/or agency standards; coordinates and monitors the delivery of waivered services to ensure the federal requirements for reimbursement are met and maintained and monitors completion of appropriate documentation; develops social service evaluations and service recommendations; oversees and supports continuity of care and waiver benefits; monitors development and ensures maintenance of individual files including required documentation; provides information and support to individuals and families in obtaining and maintaining social services benefits to ensure continued waiver eligibility; provides guidance and support to individual and/or family members in locating appropriate providers in the community; convenes, chairs and facilitates interdisciplinary/planning support team meetings to develop, review and/or modify individual service plans; coordinates integration of day program, residential, medical and other services provided to each individual and ensures service delivery; assists with identifying the need for evaluations, scheduling of evaluations and monitors completion; acts as liaison and provides technical assistance to service providers and monitors compliance with departmental and waiver policies and procedures; maintains regular contact and provides supportive guidance to assigned individuals and their families; gives information to individuals and/or call-in's, guardians and families regarding their legal rights, departmental policies and procedures, services provided and encourages participation in service planning process; schedules program reviews and monitors implementation of specific program recommendations; ensures that legal and financial documents are completed in a timely manner; informs appropriate supervisor and regional administrative staff when services are not or cannot be provided; performs related duties as required.


May review individual service plans for conformity to federal regulations; assists with responses to ICF surveys, inspections and IPR/UR as required; may coordinate the development and assist with the implementation of Plans of Correction; performs related duties as required.



KNOWLEDGE, SKILL AND ABILITY



Considerable knowledge of services available toindividuals with intellectual disability; knowledge of residential programs forindividuals with intellectual disability; knowledge of interdisciplinary approach to program planning; knowledge of intellectual disability, causes and treatment; considerable skill in facilitating positive group process; interpersonal skills; oral and written communication skills; considerable ability to understand and translate clinical findings and recommendations into program activities and develop realistic program objectives; ability to collect and analyze large amounts of information; ability to utilize computer software.



MINIMUM QUALIFICATIONS - GENERAL EXPERIENCE



The General Experience is defined as one of the following:




  1. A Bachelor's degree that meets the eligibility criteria for certification/designation as a Qualified Intellectual Disabilities Professional (QIDP) as set forth in federal regulations and interpretive guidelines and two (2) years of professional experience involving responsibility for developing, implementing and evaluating individualized programs for individuals with intellectual disabilities in the areas of behavior, education and rehabilitation. OR




  2. A Master's degree that meets the eligibility criteria for certification/designation as a Qualified Intellectual Disabilities Professional (QIDP) as set forth in federal regulations and interpretive guidelines and one (1) year of professional experience involving responsibility for developing, implementing and evaluating individualized programs for individuals with intellectual disabilities in the areas of behavior, education and rehabilitation.


NOTE: A degree that meets the eligibility criteria for certification/designation as a Qualified Intellectual Disabilities Professional (QIDP) is a degree in the field of human services, healthcare or education including but not limited to: nursing, psychology, rehabilitation counseling, special education or sociology.



PREFERRED QUALIFICATIONS



Preference will be given to candidates who have:



  1. Considerable knowledge and experience identifying and accessing state, federal, and/or community resources available to individuals with intellectual disabilities. Must have knowledge of state and federal benefits.

  2. Considerable skills and experience with leading meetings and conflict resolution.

  3. Considerable knowledge and experience with the concepts of Person-Centered Planning, as well as translating clinical assessment tools and programmatic findings into realistic and measurable objectives.

  4. Considerable knowledge of Abuse and Neglect Reporting, Notification and Follow-up to Immediate Protective Service Plans. Experience in providing critical decisions regarding individual's health and safety.

  5. Experience with self- determination, self-directed supports and self- advocacy.

  6. Strong experience problem solving and developing of creative solutions while working independently.

  7. Computer and software proficiency along with good oral and written communication skills, with experience organizing and concisely providing information to others.

  8. Experience explaining complex service systems to peers, families or people with disabilities.

  9. Knowledge and experience in providing culturally sensitive services to individuals and families of various socio-economic and cultural backgrounds.




SPECIAL REQUIREMENTS





  1. Incumbents in this class may be required to possess fluency in a foreign language or sign language for designated positions.




  2. Incumbents in this class must be eligible for certification as a Qualified Intellectual Disabilities Professional as required by Federal regulations.




  3. Incumbents in this class may be required to possess and retain a valid Motor Vehicle Operator's license.




  4. Incumbents in this class may be required to travel.
