Job Summary

Reports to the Social Work Supervisor. In partnership with nurses in the Care Management Department, social workers' focus is to provide support to patients and families. Completes psychosocial assessments which enable them to address a range of issues including adjustment to illness, safety and protection, socioeconomic issues, non-compliance, cultural and spiritual issues, crisis intervention and end-of-life care, and facilitating appropriate discharge planning and disposition

Minimum Qualifications *Eligible for sign-on bonuswith 1+ year of experience

• Education
  • Masters in Social Work from an accredited school of social work.

  • Hospital experience required
  
  
  • Knowledge of psycho-social implications of illness and disease on individual and family systems.
  
  
  
  


• Licensure and/or Certification
  
  
  • LCSW required
