Job Description:
Collaborate with campus nurses to complete MDS assessments, care plans and CAAs
Work with various disciplines to balance the need to maximize RUGS/Medicaid case mix with Quality Measures as they relate to the 5-star program
Have a thorough understanding of PDPM and assist in transitioning to this model
Develop concise and individualized care plans and coordinate with other disciplines while being an excellent problem solver/communicator
Review POC documentation for discrepancies and accuracy and educate staff as needed on proper documentation
Monitor and manage Quality Measures and complete related assessments
Assess and implement QAPI processes and projects
