Job Description:

Job Summary:

The Licensed Practical Nurse (LPN)'s role is to administer medications and treatments as per physician orders, assist in the supervision of nursing assistants, and complete all assigned tasks.



Essential Job Functions:

Assists with developing and implementing plans of care for residents.

Ensures that documentation meets all state/federal guidelines. Documentation indicates accuracy of charting:

Plan of Care
Interventions/Treatments
Administration of meds
Educating

Ensures all medication/treatments of assigned residents are given as ordered.

Perform and assist with direct patient care as needed.

Ensure resident has proper equipment and adaptive devices as ordered.

Co-ordinates resident care with other facility departments.

Assess and identify any changes in resident condition and needs and report to charge nurse.

Assist with educating patients and their families in proper healthcare procedures.

Complete Wellness Checks of all community members.

Communicates information effectively, both verbally and in writing, in a timely, clear, and concise manner.

Promotes effective liaison relationships with physicians.
