The Service Coordinator Intern under the direction and support of the Service Coordinator II, engages older adults in accessing information, resources and services to support aging in place in the affordable apartment setting. Builds one-on-one relationships while nourishing a positive community moral; creates mutually beneficial relationships with organizations and services of the local community; and, assists residents in navigating benefits and services application and compliance (in such areas as health care, counseling, nutrition, transportation, finance, home management, legal services), under the direction of the Service Coordinator II.
 

SCHEDULE:

• Monday: 8:30 am – 3:30 pm


• 
  Tuesday: 9:00 am – 1:30 pm
  


• 
   Wednesday: 8:30 am – 3:30 pm
  


• 
  Thursday/Friday (alternating weeks): 9 am – 1:30 pm

Service Coordinator Intern Responsibilities:

• Raises awareness and supports resident education regarding the availability of service coordination as a free and available resource in the building.

  Develops and demonstrates a working knowledge of benefits, services and resources available to low-income older adults.

  Provides in-person assistance to residents seeking information and assistance by phone, in the resident apartment, or a community/office space.

  
  


• 
  

  Refers resident directly to Service Coordinator for assistance with complex situations or requests, or for follow-up as needed

  
  


• 
  

  Provides documentation to Service Coordinator regarding all resident interactions, inquiries and assistance provided.

  Supports the delivery of educational and wellness events including seeking and scheduling potential presenters and programs, encouraging attendance, preparation and set-up, introduction of guest speakers, as well as start-up support for resident-lead groups and clubs as needed.

  Serves as an on-site liaison, under the direction of the Service Coordinator II, for providers of food donations, transportation, health care, counseling and other services.

  
  


• 
  

  Provides clerical support to the Service Coordinator.

  
  


• Perform other duties as assigned.
  

 

Service Coordinator Intern Qualifications/Requirements:

• Currently enrolled in an accredited college/university or community college studying human services or a similar field.

 

Service Coordinator Intern Working Conditions/Physical Requirements:

• Intermittent sitting, standing, bending and lifting of up to approximately 30 lbs


• Able to move freely throughout the building and resident apartments including climbing stairs


• Produce case notes and enter information into required software.


• Repeating motions that may include the wrists, hands and/or fingers.


• Assessing the accuracy, neatness and thoroughness of the work assigned.


• Hearing and vision abilities within normal range (corrected) to observe and communicate with others to exchange information.
