GENERAL SUMMARY
The MMC Clinic Nurse - LPN is responsible for and assists with assessment, planning, implementation and evaluation of patient care in a professional, confidential, and caring manner. Assists with patient care and procedures. Assures that a follow up plan of care and educational needs are met and coordinates the flow of patients, phone calls, lab work, and nurse visits to facilitate quality patient care and customer service goals.

- Collects subjective and objective observation data and contributes to the assessment of the patient health condition. Documents accurately in the patient chart according to policy.
- Assists management in establishing work procedures for nursing staff and implementing patient care policy and procedures for Central City and Fullerton Clinics.
- Performs a pre-visit chart review for completeness including areas of chronic diseases management
- Performs a huddle with provider regarding identified questions, individual patient care and wellness needs.
- Performs basic clinic nursing skills and administers medication and refills according to the policy and procedure.
- Works efficiently and closely with other departments for the care and treatment of the patient.
- Assists in identifying teaching, learning, and discharge needs and communicates with the provider, patient, family, Utilization Review (UR) and Social Services Designee
- Keeps flow of patients in accordance with time and appointment in order to avoid delays for the provider and undue waiting for the patient.
- Receives, investigates and reports on complaints and inquiries regarding patient issues/concerns.
- Ensures patient care area and equipment is used and correctly maintained in working condition for the safety of staff, patients and visitors.
- Anticipates patient needs and modifies work to ensure smooth patient flow.
- Complies and keeps current with accreditation standards and federal, state and local regulatory requirements
- Performs other related projects and duties as assigned.
EDUCATION AND EXPERIENCE:
Graduation from an accredited professional nursing program required. Current Licensed Practical Nurse License in the State of Nebraska required. One (1) year clinic experience preferred.
OTHER CREDENTIALS / CERTIFICATIONS:
Basic Life Support (CPR) certification required. Bryan Health recognizes American Heart Association (for healthcare professionals), American Red Cross (for healthcare professionals) and the Military Training Network.
