Responsibilities

A day in the life of a Clinical Coordinator at Hackensack Meridian Healthincludes:

    Conduct patient assessments, interventions, and evaluations for pediatric palliative care patients, as needed
  • Facilitate patient palliative care referrals as appropriate

  • Participate in family meetings and family centered rounds, as needed

  • Consult with palliative care physicians or designees, as needed

  • Maintain communication with attending physicians, residents, consulting physicians, nursing staff and other service providers,
    as needed

  • Participate in bi-weekely palliative care team rounds

  • Provide bereavement support, as needed

  • Develop, plan and coordinate patient, staff and community education regarding palliative care issues; assist in planning and
    coordinating educational activities

  • Participate in external regulatory agency survey activities (JCAHO, etc)

  • Assist with Quality/Performance Improvement activities


Qualifications


  • Current and valid New Jersey Registered Nurse license required

  • BLS/CPR certification by American Heart Association required

  • At least 2 years of nursing experience required

  • Pediatric nursing experience preferred

  • National certification preferred

  • BSN preferred
