The Clinical Case Manager provides mental health case management, including (but not limited to): identifying community/regional referral resources relevant to the needs of the students, developing relationships with community agencies, facilitating referrals for students in need of assistance, and counseling students.
DUTIES
Provide professional counseling services for UHD students, including individual, group, crisis, and occasional couples counseling
Provide brief initial assessment (triage) and urgent care coverage and appropriate referral resources to students, faculty, and staff
Initiate Counseling programming and partnerships
Support clinical staff by providing direct service and coordination of care services to students who are referred to community providers
Provide referrals to community resources and follow-up as needed
Maintain a database of local and regional referral resources and serve as a liaison to community mental health providers (e.g., establish and maintain relationships, ensure regular meet-and-greets, etc.)
Engage in timely collaboration and consultation with various university departments and offices, as appropriate, to support and advocate for students
Respond to early alert referrals requiring counselor intervention and work with faculty to assist students experiencing academic or personal challenges

Knowledge of Counseling Theory Skills: Candidates must have strong analytical and problem-solving skills, as well as excellent interpersonal and communication skills

Required Experience: Minimum of two (2) years of job-related experience
License/Certification:
