Description

A. ASSESSMENT AND PLANNING:

1. Completes patient's health history by phone interview or in person

B. IMPLEMENTATION:

1. Collects pertinent patient information

2. Documents patient health information and history in EMR

3. Schedules and orders pre-op testing as indicated

4. Provides pre-op education specific to preps, fasting, preadmission medication,

arrival and admission practices

5. Compiles the patient record, contacting physician offices as needed

6. Disseminates patient information to appropriate stakeholders for blood

transfusions, auto units, bone marrow aspirations, and medications, etc.

7. Obtains consent for treatment for patients unable to give their own consent

A. EVALUATION

1. Collects and reviews completed lab tests and EKG results prior to procedures

2. Contacts physician when indicated


Qualifications

Required:


  • Associate's degree in nursing

  • At least 3-5 years of experience in patient care in a hospital setting

  • Current RN license in the state of Illinois

  • Experience in utilization management activities (e.g. review and assessing for meeting medical necessity criteria and appropriate status)

  • Admissions and discharge planning and/or case management

  • Critical thinking, decisive judgement, problem solving skills, and the ability to work with minimum supervision.

  • Strong foundation of clinical knowledge.

  • Ability to prioritize and demonstrate efficiency with job duties.

  • Outstanding interpersonal, communication, and customer service skills.

  • Ability to learn computer skills in Microsoft Word, EMR, Midas systems within 30 days.

  • Able to multi-task quickly and process accurately

Preferred:

  • Bachelor of Science in Nursing
