Works to implement the rehabilitation activities outlined in the individual's IRP to address skill building and enhancing peer and family relationships , emotional management, social, self-care, independent living skills, and activities that support the individual's or youth's cultural interests. These include but are not limited to; enhance community functioning in identified clients (adults, youth) by focusing on activities of independent living related to resilience, recovery and rehabilitation. Efforts and interventions are integrated with those of the primary therapist and other relevant health care providers.
MINIMUM QUALIFICATIONS / REQUIREMENTS:
• Bachelor's degree in Social Work, Psychology, or related field plus One-Two years of work experience with youth and/or adults with serious and persistent mental illness, or AA degree in related field plus 4 years work experience.
• Must be able to move freely between locations which may or may not be accessible to individuals with physical disabilities. Position requires intermittent standing, sitting, walking and physically capable of lifting and moving files/supplies/equipment up to 25 lbs. Individual requires sufficient visual and hearing acuity to perform duties of position.
