Overview

The Academic Health Care Center is an ambulatory health care teaching institution under the auspices of the New York Institute of Technology College of Osteopathic Medicine(NYITCOM) in Old Westbury, New York. NYITCOM ambulatory health system is comprised of two health centers, one in Old Westbury (Academic Health Care Center) and one in Central Islip, NY (Family Health Care Center).

The function of the academic health centers is to provide primary medical care and specialty services to students, staff, faculty, and the general public. As part of the medical school, the health centers serve a major role as a vehicle for medical students to gain exposure to delivering medical care at an outpatient site. The variety of clinical specialties in which students can learn include family and sports medicine, osteopathic manipulative medicine, pediatrics, neurology, behavioral health and internal medicine. In addition to medical care the health centers also operate a rehabilitation center which includes physical therapy, speech therapy and occupational therapy.

NYIT's College of Osteopathic Medicine seeks a full time Patient Navigator for the Family Health Centers at our Central Islip and Old Westbury locations.

The Patient Navigator provides a holistic approach to care for patients with complex needs, especially for those who struggle with chronic conditions. The Patient Navigator provides personal guidance to patients as they move through the health care system. The Patient Navigator works with patients and their families to help with many different needs associated with the health care system. This may include helping with insurance challenges, finding doctors, explaining treatment and care options, scheduling doctor's visit, communicating with their health care team, assisting caregivers, and managing medical documents. The Patient Navigator will be based at both Health Care Center locations in Old Westbury and Central Islip.

Responsibilities

Specific Responsibilities:

    Referral to community resources as needed.
  • Assistance with medical transportation.

  • Arrangement for durable medical equipment (DME) and ancillary services as needed or ordered by care provider.

  • Outreach to patients to promote assistance with keeping doctor's appointments.

  • Provide navigation for patient by addressing any logistic barriers, scheduling complications, etc., that would prevent a patient from showing up for their appointment.

  • Facilitating communication between the patient, family members and health care providers to ensure patient satisfaction and quality of care.

  • Work with patients and their families to identify and address barriers to seeking health care and following their medical treatment plan of care.

  • Arrangement of home care services.

  • Assistance with health insurance questions and concerns.

  • Informing the patient of social and financial aspects of care and linking patients to resources as needed.

  • Assuring that their health insurance is active and if not assist the patient in process.

  • Assisting patients with changing PCPs when required by insurance when patient is unfamiliar with making the needed changes in order for the patient to proceed with seeing the physician.

  • Assisting with intake paperwork or any paperwork needed prior to seeing the doctor.

  • Intervening when patients facing barriers, such as language, which may hinder continuity of care.

  • Develop initiatives in support of patient needs as per the recommendations of the physicians and approval of supervisor(s) such as the Diaper Drive and future community initiatives.

  • Schedule information sessions in Spanish /and English at the Family Health Center on a basic overview of Medicaid Community Plans most commonly used by our patients.

  • Refer to internal or external case management services when other issues are identified (i.e. hunger issues, domestic violence issues, etc.)

  • Processing referrals and pre-authorizations as needed.

Qualifications


  • A Bachelor's Degree is required.

  • Must be organized, attentive to detail and compassionate.

  • Ability to follow-through, excellent communication skills and having a good understanding of health care facilities and processes is required.

  • Bilingual and able to communicate effectively in Spanish is required.
