Job Description:

Responsibilities

Communicates relevant patient information concerning the care of specialized patient populations referred to VNSNY Home Care to VNSNY operations team as needed.

Acts as a resource towards the integration of evidence-based care practices into protocols, policies, consultation strategies, and continuous quality improvement initiatives.

Leads the care coordination between the home care and hospice services by assessing identified potential hospice patients to determine hospice eligibility and appropriateness.

Collaborates with the Coordinator of Care to navigate the transition with the patient, family, physician, and home care team through the hospice education, evaluation, and decision making.

Works under general supervision.
