Will provide a patient centered clinical component to the patient access intake process, in order to expedite the intake process. RN Intake Coord will provide a clinical review of all referrals, new and/or consults, and collaborate with providers to ensure patients are scheduled with the right provider/s in a timely manner, and will work closely with the Intake Team to ensure the necessary information and records are available for a meaningful new patient/consult visit. Oncology experience preferred.
Experience and Education
Graduate of NLN approved nursing program.
Three (3) years experience in medical intake and/or care coordination; experience in relationship development and managed care.
Current Texas (or BNE Compact State) license,
BCLS required through American Heart Association.
Job Duties
Receives and provides immediate responses to inquiries and patient referrals. Establishes immediate relationship between referral sources and hospital.
Determines appropriateness of referral for admission in accordance with established admission criteria for services involved.
Interfaces with other hospital departments and physicians to facilitate admission process.
Evaluates financial reimbursement for proposed hospitalization and intervention and obtains pre-certification for intervention.
Assists in determining resources required to provide appropriate and indicated level of care and assures availability of indicated resources prior to acceptance of patient.
Provides overview of services to be provided during hospitalization to patients, families, payers and referral sources.
Notifies payor of admission and expected course of treatment and assigns initial length of stay in accordance with standards of care criteria as outlined in utilization.
Tracks data regarding intakes and referral sources.
Provides guidelines and instruction to staff regarding program promotion, intake and referral processes.
Communicates information about patient referred to rehab unit to appropriate physician.
Accepts additional responsibilities and organizes scheduled and unscheduled time.
