Chandler Hall, a Quaker facility and Kendal affiliate, provides high-quality care for older adults in Newtown, Bucks County. Chandler Hall offers a wide spectrum of services for individuals at all stages of the aging process including: Community Living, Personal Care, Residential Hospice, Home Hospice, Independent Living, Respite Care, and Skilled Nursing Care. We are always seeking individuals with a passion for working with the older population.
We are currently seeking a P/T LPN for our Memory Care Unit. Shift consists of every Wednesday and every/other weekend.
Provide resident nursing care, medication management/supervision, treatment and supervision of direct care staff. Provides support to the Personal Care Team, including oversight/ supervision, on going education to the direct care staff, to meet the needs of the residents and their families. You are responsible for working as a team member, for assisting co-workers and managers in the pursuit of excellent service.
Responsible for executing physician orders with an understanding of why the orders were issued and with knowledge of there desired or possible adverse effects.
Records and reports pertinent observations of residents’ responses to medical, nursing and therapy to the physician and resident responsible party.
Reports complaints and concerns made by the resident, families, or staff and makes a report to the Administrator in a timely fashion.
Orders medical tests for the resident when ordered by the Physician and communicate with the resident and family when a test has been ordered.
Prepares resident for medical tests by communicating with the direct care staff and resident necessary instructions.
Regularly communicates needed information to the residents, families and resident designated responsible party.
Records admission notes, resident changes, and discharge notes in the electronic medical record
Completes incident reports when a resident has a fall, med error, skin injury, allegation of abuse in the electronic medical record.
Assists the Personal Care Administrator to obtain written witness statements when there is an allegation of abuse or injury of unknown origin.
Reports changes in the residents’ condition to the Coordinator.
Executes physician orders, accepts, accurately transcribes and carries out physician orders. Demonstrates an understanding of the reasons, desired and adverse outcomes. Demonstrates knowledge of resident monitoring related to medications and treatments, addresses concerns to the RCC for resolution.
Administer medications and performs treatments per physician order and within the scope of your practice in Pa.
Collaborates with Team Members to meet lifestyle choices and satisfaction, assist team members to resolve conflicts while meeting each resident needs or /choices.
Demonstrates knowledge of ordering, securing administration, documentation of Control Drugs to maintain resident safety, regulatory compliance with federal laws.
Assists the Resident Care Coordinator with the completion of Support Plans.
Completes Chart Checks daily and runs a missed documentation report. Communicates immediately with the Medication Care Partner if documentation is not complete.
Randomly Audit medication Carts to ensure that all medications are available and medications are not expired.
Mentors Direct Care Staff to their job responsibilities.
Supervises Direct Care Staff on your assigned shift.
Attends Personal Care Staff meetings regularly.
Assist with staff call outs by notifying the Coordinator of the call out and make calls to other Team Members to replace the staff as soon as you become aware that a Team Member is not reporting to work. EEO
