The Program Specialist/LifesharingSpecialist is responsible for ensursing programmaticregulatory compliance and documentation.Creating an environment that empowers individuals who receive or provide support, through the development of skills and coordination of resources, in accordance with Everyday Lives and Person-Centered Practices.
Oversee the quality of programmatic plans, ensuring individuals' health and safety is upheld in accordance with each ISP.
Ensure that identified services, goals, and outcomes are accurately documented, adhering to PA Chapter 6100 regulations.
Representprogram in a professional manner; working in conjunction with Supports Coordination Organizations, Administrative Entities, the Office of Developmental Programs, and other ancillary services.
Organize and maintain individual files in accordance with PA Chapter 6500 regulations and program requirements.
Complete individual assessments, SEEPs, quarterlies, and other applicable documentation, within regulatory standards.
Assist and prepare new Lifesharing Providers for the self-licensing assessment.
Provide and conduct applicable trainings for Lifesharing families and direct care professionals.
Complete visits to sites monthly, or as needed.
Maintain a minimum of 24-hours of professional and service-related trainings per training year.
Participate in supervision and professional development plans with program management.
Serve as an incident management point person for the program within EIM.
Participate in a regular on-call rotation with other team members.
At least one year case management experience in supporting individuals in a lifesharing program, supportive living, community based residential program with a focus on person centered planning.
Master's Degree from an accredited educational program with 1 years' work experience working directly with persons with an intellectual disability or autism, or
Bachelor's Degree from an accredited educational program with 2 years' work experience working directly with persons with an intellectual disability or autism, or
Associate Degree from an accredited educational program with 4 years' years' work experience working directly with persons with an intellectual disability or autism, or
Demonstrate a strong, working knowledge of DPW Chapters 6500 and 6100 Regulations.
