Job Description:


Department Description: Affiliated with the world-famous Joslin Diabetes Center, specialists in our program provide treatment for adult diabetes, nutrition counseling, and patient education. Our outpatient clinic makes it convenient for patients to receive specialized care for diabetes and laboratory services all in one location. Our center is located less than a mile from Beth Israel Deaconess Hospital-Needham at 145 Rosemary Street in Needham. This gives patients easy access to hospital-based services at BID Needham and, when needed, a quick referral to Beth Israel Deaconess Medical Center in Boston. For more than two decades, patients with diabetes have received quality care at the Joslin Diabetes Center Needham. Affiliated with the world-renowned Joslin Diabetes Center in Boston, our Center provides comprehensive evaluation and treatment of diabetes, including education for insulin pump therapy. The American Diabetes Association has recognized the Joslin Diabetes Center Needham for meeting the national standards for diabetes self-management education.

Job Summary: Certified Diabetes Educator responsible for diabetes self-management program utilizing the clinical practice recommendations of the American Diabetes Association, Association of Diabetes Educators, and Joslin Center, Needham.

Essential Responsibilities:


  1. Perform clinical and psychosocial assessment of diabetes patients in either an individual or group setting.

  2. Identify educational needs for diabetes self-management knowledge and skills.

  3. Develop and implement a plan of care based on clinical expertise, scientific principles and the patients' lifestyle and culture.

  4. Empower diabetes patients with knowledge and skills to attain optimal control and prevent acute/chronic complications.

  5. Educate in areas of medication administration, SMBG, pump therapy, nutrition, exercise, recognition & treatment of acute complications and preventive health skills in accordance with current clinical practice recommendations utilizing learning objectives and behavioral goals.

  6. Provide Available consultation to patients daily as needed. (Pager availability).

  7. Monitor patients' response to therapy and revise plan as indicated.

  8. Prevent/decrease incidence of complications by supporting preventive health risk behaviors.
    (Ophthalmology, podiatry, lipids, A1C, urine M/A)

  9. Conduct ongoing quality improvement studies.

  10. Provide opportunity for diabetes support group sessions.

  11. Has the authority to direct and support employees daily work activities. Has the direct responsibility to undertake the following employment actions: hiring, termination, corrective action and performance reviews. Direct Reports: 4-6 Indirect Reports: None


  12. Has full responsibility for planning, monitoring and managing department budget.



Required Qualifications:

  1. Bachelor's degree in Nursing required.

  2. License Registered Nurse required., and Certified Diabetes Care and Education Specialist (CDCES) credential.

  3. 5-8 years related work experience required in direct diabetes education. and 0-1 years supervisory/management experience required

  4. Experience with computer systems required, including web based applications and some Microsoft Office applications which may include Outlook, Word, Excel, PowerPoint or Access.

  5. Oral Communications: Ability to comprehend and communicate complex verbal information in English to medical center staff, patients, families and external customers.


  6. Knowledge: Ability to demonstrate in-depth knowledge of concepts, practices and policies with the ability to use them in complex varied situations.


  7. Team Work: Ability to act as a team leader for small projects or work groups, creating a collaborative and respectful team environment and improving workflows. Results may impact the operations of one or more departments.


  8. Customer Service: Ability to provide a high level of customer service and staff training to meet customer service standards and expectations for the assigned unit(s). Resolves service issues in the assigned unit(s) in a timely and respectful manner.

Physical Nature of the Job:
Light work: Exerting up to 20 pounds of force frequently to move objects. Some elements of the job are sedentary, but the employee will be required to stand for periods of time or move through out the hospital campus
