We have an open position for a Hospice Registered Nurse to work full time 40 hrs. a week caring for our hospice patients in their own homes and facilities where we provide hospice care. The RN will work collaboratively with a team of clinicians responsible for end of life care for our patients.

Hospice Registered Nurse Job Qualifications:

Duties include:


  • Assess the patient and follow a comprehensive plan of care for nursing

  • Provide ongoing assessment/observation of patients, assuring an accurate evaluation of patient needs

  • Maintain current and accurate patient records

  • Have good organizational and communication skills

  • Be able to function independently in a home setting with minimal supervision

  • Be able to maintain relationships and communication with patients, families, and care team

  • Case management

  • Certification in Hospice and Palliative Care, Oncology or end-of-life experience is preferred, but willing to train the right candidate

  • Excellent communication and computer skills required

  • NH RN license and CPR certification required

  • Experience with EMR, we use Netsmart formerly Allscripts

  • 1 - 2 years: oncology, palliative care or end of life experience certification in hospice palliative care RN a plus!
