We have an open position for a Hospice Registered Nurse to work full time 40 hrs. a week caring for our hospice patients in their own homes and facilities where we provide hospice care. The RN will work collaboratively with a team of clinicians responsible for end of life care for our patients.
Hospice Registered Nurse Job Qualifications:
Duties include:
Assess the patient and follow a comprehensive plan of care for nursing
Provide ongoing assessment/observation of patients, assuring an accurate evaluation of patient needs
Maintain current and accurate patient records
Have good organizational and communication skills
Be able to function independently in a home setting with minimal supervision
Be able to maintain relationships and communication with patients, families, and care team
Case management
Certification in Hospice and Palliative Care, Oncology or end-of-life experience is preferred, but willing to train the right candidate
Excellent communication and computer skills required
NH RN license and CPR certification required
Experience with EMR, we use Netsmart formerly Allscripts

1 - 2 years: oncology, palliative care or end of life experience certification in hospice palliative care RN a plus!
