Job Description:

Lakeview Center is best known for its caring, compassionate treatment for people with mental health issues, drug and alcohol dependencies, or intellectual disabilities. The breadth and depth of these services are the most comprehensive in the region. They range from inpatient and residential treatment to a gamut of outpatient programs including trauma services, psychiatry, counseling, day treatment, and intensive, round-the-clock outpatient services to help people with severe mental illnesses safely remain in the community. We reach people of all ages from babies and children to adolescents and adults. In all, we offer more than 60 programs and services for the people of Northwest Florida.

The Adult Community Team/LEAP Team (Linking, Engaging Advocating and Assessing plus Planning) combines a myriad of very intensive, unique, and specialized services designed to disrupt the cycle of high cost utilization and divert into more traditional modalities of outpatient service provision. The backbone of this project is an Intensive Case Management Type Team; a multidisciplinary team designed to provide assertive mental health, substance abuse, and care coordination services to the population of individuals that disproportionately use the most intensive and highest cost services as their primary care. Provision of medical care will be accomplished via community linkages with the local Federal Qualified Health Center (FQHC), and private practice offices for health care needs. The overall objective is to increase ongoing adherence with medical and behavioral treatment, thereby reducing the number of emergency room visits, crisis stabilization and hospital admissions, and incarcerations or interactions with law enforcement.
