Job Summary

Emerson Rehabilitation and Transitional Care Unit (TCU) is a 20 bed hospital based short term rehab unit, licensed as a skilled nursing facility, located within Emerson Hospital in Concord, MA. We are a 5-star facility for health inspections, staffing, and quality measures.

Qualifications/Requirements:


  • Seasoned, skilled professional with high standards for quality patient care

  • BSN required with MA RN license

  • DON experience in a SNF with a minimum of 5 years of experience in a management capacity, in addition to an acute care background preferred

  • Knowledge required in the areas of DPH regulations, Medicare, Managed Care, MDS, Reimbursement, Budgeting, and Finance

  • Ability to collaborate with hospital departments providing services to the TCU

Reporting to the Associate Chief Nursing Officer, assumes 24-hour responsibility for management of their unit(s) and allocation of resources. Management includes provision of appropriate staffing and skill mix, and sufficient equipment and supplies to meet the nursing care needs of the patient population. Directs the quality and continuity of patient care on their unit(s) in order to provide comprehensive care appropriate to the type and age of the population served.

Responsible to the patient and the family or significant others, medical staff and hospital administration for the quality and cost effectiveness of patient care delivered.

Job Functions


  • Develops, monitors and evaluates clinical and patient satisfaction initiatives, standards of nursing practice and outcomes of patient care.

  • Develops and monitors department budgets to adhere to financial objectives of the organization.

  • Participates in the development of organizational policies, procedures and standards which direct patient care and describe the expectations of nursing staff.

  • Assumes or delegates responsibility for counseling, recognizing and evaluating staff regarding their job performance.

  • Provides access to an appropriate level of care for patients in the institution by effectively managing patient flow between units or services in collaboration with physicians and staff.

  • Acts as a community liaison and positive representative of the institution in the community.

  • Assesses and plans for staffing needs to meet patient care needs.

  • Maintains clinically current to positively affect nursing practice.

  • Collaborates with nursing leadership to develop plan of care for department.

  • Represents the organization in a professional manner in communication, attire and interaction with others.

  • Maintains and improves level of practice through hiring and evaluation practices.

  • Collaborates with all health care providers to ensure best practice methods.

  • Responds to issues and develops a plan to resolve or rectify patient, family, staff concerns.

  • Communicates effectively with all disciplines.

  • Supports CQI objectives of organization through interdepartmental efforts as well as unit initiatives.

  • Implements plans to meet regulatory standards.

  • Assesses learning needs of self, staff and unit as well as develops a plan to meet the educational needs to meet standards of care practice.

  • In collaboration with nurse educators, provides staff education on an ongoing basis. This includes new policies, Joint Commission standards, new equipment/products and clinical software programs.

  • Supports professional growth and development of staff.

  • Supports collaboration through interdisciplinary committee participation.

  • Fulfills responsibilities as the administrative, house supervisor, as assigned.

  • Maintains and supports patient's rights to care, privacy and respect.

  • Other duties as assigned from time to time.

Minimum Qualifications

Education


  • Graduate of a professional nursing school.

  • Bachelor's degree in nursing required.

  • Masters degree in nursing or related field such as public health, healthcare administration, or business preferred.

Experience

  • Minimum of five years experience in a leadership position required.

Licensure and/or Certification


  • Currently licensed to practice professional nursing in Massachusetts.

  • BLS healthcare provider required.

  • Certification in clinical specialty preferred.

Skills


  • Demonstrated leadership ability, financial management, and strong interpersonal and communication skills.

  • Excellent customer service skills.

  • Must be able to read, write, and communicate in English.

  • Computer skills required (including MS Word, Excel, PowerPoint).

  • Ability to maintain confidential information.
