SUMMARY
The primary responsibility of the Nurse Care Manager is to telephonically design and oversee the delivery of care plans for each patient in their panel. Serving as the primary point of contact for their patients, the Nurse Care Manager partners with all members of the Complex Care Management (CCM) team by gathering clinical input, facilitating team discussions, and coordinating all elements of their patients’ care plans. The Nurse Care Manager is also the telephonic central point-of-contact for their assigned PCP’s, maintaining positive relationships between the PCP’s and Tandigm Health. Position is remote after training.

· Minimum 3-5 years of case management (CCM certification preferred), disease management, utilization review (guideline knowledge preferred), and transition of care experience is required

PHYSICAL REQUIREMENTS IN ACCORDANCE WITH ADA
· Carrying/Lifting: Occasional, 0-25 lbs.
· Standing: Frequent, up to 6 hours per day
· Sitting: Frequent, up to 6 hours per day
· Walking: Frequent, up to 6 hours per day
· Repetitive Motion: Keyboard activity, telephone use, writing, reaching, grasping, pushing, pulling
· Visual Acuity: Ability to view computer monitors and read newsprint, with or without corrective lenses
· Environmental Exposure: None
