Job Description:


The Community Builder seeks a highly motivated candidate with a passion for direct services to coordinate the implementation of the TCB housing and health initiative in Oakwood Shores. As a housing provider, TCB is uniquely situated to support our residents because we have formed deep relationships with families throughout the past decade.


The Health Navigator will work alongside resident leaders to:




  • Increase chronic disease self-management with home community-based practices.






  • Increase housing stability






  • Increase access to healthy food options for the community






  • Increase care coordination for residents experiencing chronic diseases.






  • Increase resident leaders in the community and connect them to potential employment opportunities in health-related fields




The ideal candidate will have a strong academic background, research experience, and an interest in working directly with inner city communities, and families. The Navigator works to establish communication, relationships and maintain mutual understanding around health in the community. The role is a comprehensive system effort to increase access to healthier options in the community as develop resident leaders who will infuse healthier options into the Oakwood Shores culture.


Under the direct supervision of the Community Life Senior Manager, the Health Navigator will be responsible for working with resident leaders to bring programs to the community that increase the health of residents overall.


Essential Functions:




  • In conjunction with Resident Leader [ Health Champions] fulfill all components of the LinC Grant






  • Recruit and train 5 Health Champions






  • In conjunction with Senior Manager, host Health Champion Orientation /manage HC payroll processes






  • Create programming that connects families to healthy, affordable food options and support TCB's smoke-free housing policy






  • Connect TCB residents to health care and screening services.






  • Host & Participate in all Oakwood Shores health related events.






  • Create Intergenerational program opportunities that remove barriers to resources for residents






  • Submit Monthly report on Linc Efforts/Present virtually at monthly neighborhood meeting




  • Document all efforts in CL Dynamics





Basic Qualifications:




  • Experience in community-based education/social-work interventions






  • Experience in grassroots community outreach and organizing






  • Knowledge and experience of working in diverse and low-income communities






  • Capable of providing confidential support to residents






  • Excellent organizational, communication (oral and written), and problem-solving skills






  • Demonstrates organizational skills, flexibility, assertiveness, and team orientation
