Patient Navigator - REMOTE / Work From Home - Referral Management

Position Highlights

    • Work location: This position is a Remote / Work from Home Opportunity
      • Travel required 2 days or more monthly in office at SWHR Corporate Headquarters, 1603 LBJ Freeway, Farmers Branch, Texas 75234

    • Work environment: Referral Management
    • Work hours: Full-time, 40 hours weekly, Monday through Friday, 8:00am - 5:00pm
      • 6 weeks Training, Monday through Friday, 8:00am - 5:00pm

    • Salary range: $15.70 - 23.17 per hour (based on relevant experience)

    • Please note: The Patient Navigator is employed by SWHR. SWHR is a clinically integrated network formed by Texas Health and UT Southwestern.

Qualifications

    • High school diploma or equivalent (required)
    • 6 months Healthcare Scheduling or Health Care Office Experience (required)
    • Referral Processing or Scheduling Experience (preferred)
    • Demonstrated knowledge of Intermediate Computer Skills (preferred)


Position Responsibilities SWHR Patient Navigator is a critical role in ensuring that individuals in our community are provided with every opportunity to have all the necessary care required for themselves and their family. These individuals make a difference by guiding patients through the complex medical system by processing and completing referrals as directed by Primary Care Providers, following Patient Navigation protocols and expectations, utilizing exceptional customer service skills to communicate with patient, referring and service providers, and clinic staff to complete referrals accurately and expediently to minimize errors and reduce delays in care. Document referral information in tracking program to be consistent with documentation in the EMR and communicates appropriately with all stakeholders. Messages primary care providers or designated clinic staff any and all patient care related concerns or relevant information, redirecting clinical conversations to clinical personnel, utilizing Patient Navigation team for assistance to complete referrals by utilizing appropriate communication forms and templates. Submits documentation to obtain insurance referral authorizations and pre-certification for ancillary services, diagnostic testing and imaging and coordinates pre-admission testing requirements with clinic personnel and patient. May educate patient on expectations and follows up on set tasks and pending or incomplete referrals.

In addition to the required qualifications, a successful Patient Navigator will:

    • Have exceptional customer service skills

    • be accurate, efficient and dependable
    • have strong verbal and written communications with all levels of people; bi-lingual a plus
    • demonstrate good judgment
    • consults with others and shares knowledge
    • have computer skills using Microsoft Excel, Word, and Outlook
