Southwestern Health Resources Clinically Integrated Network (SWHR CIN), is hiring for a Director of Quality Outcomes and Performance Improvement. This position will work under limited supervision to execute strategies to lead Southwestern Health Resources' comprehensive performance improvement program for the physician network.

Location: Farmers Branch

Job Description

Initiates and oversees the development and deployment of a comprehensive performance improvement program for the primary care or specialty care network of SWHR. Includes value stream program development, data analysis/reporting, regulatory data submission (MIPS, Commercial/Medicare Advantage ACO and Next Generation ACO), and identification of Network opportunities for performance improvement. Incorporate elements of Lean, Six Sigma, and Project Management to lead a team that drives financial, clinical and operational improvements across the Network.

Responsible for planning, directing, and coordinating activities to monitor and initiate improvement strategies to meet quality and performance indicators for all contracted SWHR network lives including Electronic Medical Record (EMR) optimization, business intelligence tools design and optimization, and practice transformation of patient centered care.

Designs and oversees the operations of development, maintenance and expansion of the Quality Dashboards and provider reports required for the SWHR network providers to meet contractual obligations, incentive programs and quality performance. Ensures the accuracy of the reporting and business intelligence tools with analytics across a vast network of disparate technologies, platforms and accessibility.

Designs and implements, in conjunction with the Chief Quality Officer and VPs Quality, the infrastructure, strategy and annual work plan to ensure the success of SWHR's Quality, Outcomes and Performance Improvement program, including establishing annual goals and objectives, as well as the relevant roles and responsibilities in a multi-disciplinary collaborative environment.

Collaborates with colleagues (analytics, IT, referral management, care management) across SWHR to ensure all requirements related to quality are being met and updated annually into the processes and workflows of the key programs. Performs root-cause analysis and other problem-solving activities to identify effective corrective actions and process/performance improvements.
Provides strategic oversight and contributions to clinical quality committees.
Effectively leads and mentors a team of performance improvement trained experts to achieve the quality goals of the organization.
Prepares reports and analysis of clinical outcomes data in order to identify opportunities for clinical process improvement initiatives.
Motivates and values staff around performance excellence, professional development, and participation in meetings and committees.
Prepares written and oral presentations for a variety of internal and external audiences.
Performs other duties as assigned.

Education
Bachelor's Degree in healthcare or engineering required. Or, Master's Degree in healthcare or engineering is preferred

Experience
8 years experience in quality improvement/performance improvement as a primary role with a minimum of (3) three years supervisory experience (with Bachelor's degree) required. Or, 7 years experience in quality improvement/performance improvement as a primary role with a minimum of (3) three years supervisory experience (with Master's degree) required.

Licenses and Certifications
SSBBC - Six Sigma Black Belt Certification required upon hire, Or, SSMBBC - Six Sigma Master Black Belt Certification required upon hire

Skills
In-depth knowledge of Six Sigma DMAIC, DFSS, system engineering, IHI Model for Improvement, Theory Constrains and/or Theory of Profound Knowledge.
Ability to develop relevant KPIs and metrics in alignment with organization strategic plan.
Analytical skills to differentiate between special cause and common cause variation.
Ability to see boundaries and care setting which a service and/or information value stream flows.
In-depth knowledge with the human side of change and Tuckman's model of team development.
Strong change management and facilitations skills.
Ability to function independently with limited or minimal supervision.
Ability to effectively collaborate with individuals at all levels of the organization.
Ability to prioritize and organize large scale projects across the health system.
Ability and skills with A3 management process and quality improvement tools, such as Microsoft Project, Visio, Excel, SharePoint, or Minitab.
