Family Wellness Program Coordinator at Glade Run Lutheran Services in Zelienople, Pennsylvania

Job Description:

The Family Wellness Program Coordinator will work within Glade Run's Family Wellness and Jeremiah Village programs with approximately 75% of the time spent in Family Wellness and 25% in Jeremiah Village duties.

The Family Wellness Program will provide safe, sound housing and support to help families work through difficult situations to achieve a better life. Case management is the main service component of the Family Wellness Program.  It serves as a means for achieving client wellness and autonomy through advocacy, communication, education, identification of service resources and service facilitation. Throughout the program, the parent will continue to work on their identified goals with the ultimate goal of moving out of supportive housing into a permanent living space of their own.  

Jeremiah Village is a brand new inclusive, mixed ability, mixed income supportive housing community located on Glade Run’s historic Zelienople campus. TREK Development Group will manage the property and Glade Run will provide enhanced supportive services to residents.  Specifically, the Family Wellness Program Coordinator will fulfill a supportive services role within Jeremiah Village by creating opportunities for residents to interact with each other and the community, improving residents’ ability to uphold lease obligations to decrease turnover, improve access to services, and support resident-led initiatives that promote shared social responsibility for the quality of life in the development. 

QUALIFICATIONS


  1. Bachelor’s Degree required, Master’s degree preferred, with major course work in sociology, social welfare, psychology, gerontology, anthropology, criminal justice, criminology, theology, nursing, counseling, education or other related social science or human services field.

  2. One to three years working with families in a mental health setting required.

  3. One to three years case management experience required.

  4. At least one year supervisory experience. 

REQUIREMENTS


  1. Must possess excellent verbal and written communication skills.

  2. Must have a valid driver’s license, acceptable driving record, and access to a reliable automobile.

  3. Ability to adapt hours to needs of position.  May need to work evenings and/or weekends hours periodically including providing on-call emergency services on a rotating basis.

  4. Access to a telephone.

  5. Ability to remain organized despite pressured situations.

  6. Must be eligible for PA State Police clearance, PA Child Abuse History Clearance, FBI Clearance, Arrest/Conviction Report and Certification Form, and Act 168 Forms. 

  7. Must be able to identify with the agency’s Mission Statement and support the agency’s Core Values.

CORE COMPETENCIES:

Family Wellness Program Coordinator Duties:


  1. Obtain and complete the service history information, initial assessment material, encounter forms, service documents, and the individualized service plan in a timely and  accurate manner.

  2. Assist consumers with accessing appropriate community services, living in the least restrictive setting, obtaining basic necessities, and conducting activities of daily living.

  3. Provide on-call services as assigned and develop crisis plans for assigned consumers.

  4. Ensure the supervisor is informed regarding consumers, as well as accepting and utilizing routine supervision.

  5. Develop and maintain professional relationships in the community and with other office staff.    

  6. Provide services at the level indicated by the consumer's individualized service plan and outlined by the pertinent regulations.

  7. Complete discharge documentation in a timely and accurate manner

  8. Liaison with County CYS to accept referrals and facilitate agency admissions, transfers and discharges

  9. Make day to day decisions regarding cases in the program and keep the Director of Family Based MHS informed of high-risk cases, pertinent issues related to clients and contracting agencies.

  10. Carry a full caseload and provide direct care in accordance with the following requirements; Develop Service Plans in collaboration with families, county and community representatives. Complete Service Plans in a timely fashion in accordance with regulations and agency policies.  Involve family members and stakeholders in the service planning process and indicate participation through obtaining signatures of all participants.

  11. Complete a psychosocial history for all clients within 30 days of admission to Glade Run Lutheran Services. Conduct interagency meetings as needed or required by agency’s policy. Access and link families to needed community resources. 

  12. Provide discharge planning and follow-up, beginning with planning at the time of admission. Submit required information for program database in a timely and thorough manner.

  13. Complete all required paperwork recognized in the designated record set within 30 days of admission.

  14. Will work collaboratively with families in order to provide families with needed support and education related to their individualized needs including childcare and  transportation.

  15. Will work cooperatively and in conjunction with all Family Case Managers to ensure the needs of the families are met.

  16. Provide on-going supervision and support to family case managers.  Weekly supervision to include review of cases, service planning, thorough and timely completion of case documentation.  Provide supervisory back up to staff on call and unscheduled supervisory consultation when needed by staff. Maintain appropriate documentation on supervision.

  17. Implement monitoring tools to ensure supervisees’ use Glade Run values in service delivery.

  18. Coach staff to assist families to identify team/support people and that each individual in the family is involved in decision making.

  19. Fiscal oversight and monitoring costs of program

Jeremiah Village Program Coordinator Duties:


  1. Fulfill supportive services role within Jeremiah Village by creating opportunities for residents to interact with each other and the community.  

  2. Improve residents’ ability to uphold lease obligations to decrease turnover. 

  3. Improve access to services. 

  4. Support resident-led initiatives that promote shared social responsibility for the quality of life in the development.  This will be done within the set social/education framework of hosting six Community Network Nights and two social/recreational events annually, recruiting and supporting resident leaders to be stewards.  

  5. Host new resident orientation quarterly. 

  6. Provide a resident Newsletter. 

  7. Process management referrals of residents who have lease violations (housekeeping, late rent, etc.).  

  8. Provide outside referrals and support to residents. 

  9. Collaborate with management to attend move-in, recertification, and apartment inspections when possible.
