Job Description:

POSITION PURPOSE:
The Clinical Manager works under the direction and guidance of the Branch Administrator to provide
supervision of all clinical staff working with high medical acuity clients and/or clients receiving extended non‐licensed support services. The Clinical Manager is responsible to provide staff orientation and ensure competence as a Preceptor and or ensure RN Case Manager/Preceptor resources are in place. Ensures that services are delivered according to licensing guidelines, professional medical standards standards and agency policies and procedures. Upholds the Recover Health Mission in every Interaction.

MAJOR RESPONSIBILITIES/JOB FUNCTIONS
Standard: Supervising Physician or Registered Nurse
The skilled nursing and other therapeutic services furnished are under the supervision and direction of a physician or a registered nurse who preferably has at least 1 year of nursing experience and is a public health nurse. This person (or alternate) is available at all times during operating hours and participates in all activities relevant to the professional services furnished, including the development of qualifications and the assignment of personnel.

Is appointed by the Branch Administrator in consultation with the Chief Operating Officer (as part of the Governing Body)
Plans, develops, and manages the clinical programs, services, activities, and employees of the agency consistent with company policy and regional management directives.
Assists with the oversight of the agency’s growth related to home care.
Serves as the clinical expert and assists with the operational and financial management of the agency.

Reports to: Branch Administrator
Leadership, Agency Development & Financial Management Leadership
1. Applies critical thinking skills to facilitate leadership, problem solving, and strategic planning related to operational processes and clinical services.
2. Establishes and documents designee in the absence of the Clinical Manager.
3. Is flexible with schedule and is readily accessible for agency needs.
4. Investigate and take appropriate actions on client/consumer complaints.
5. Participate in the recruiting, hiring, and identifying the training needs of clinical staff
6. Perform annual employee evaluations for all clinical staff.
7. Maintains ongoing liaison between the Administrator and the staff.
8. Available as needed for emergent situations, surveys, and clinical guidance for staff.
9. Participates in hiring of qualified personnel and ensures appropriate orientation for all personnel.
10. Identifies the standards of practice for agency personnel and communicates these standards to them.
11. Oversees and/or provides staff development and inservice education programs to ensure competencies of
staff and to meet regulatory requirements. Develops systems, which support the ongoing professional development of the agency staff.
12. Provides staff direction through general meetings, timely personnel evaluations, and identified performance expectations.
13. Provides general oversight and leadership to all office staff.

Agency Development
1. Participation with the Professional Advisory Board. Investigates and makes recommendations to the Advisory Committee for future growth of all home health care services and products.
2. Ensures the accuracy of public information materials and activities.
3. Participates in the organization's strategic planning activities if requested.
4. Assures accurate implementation of a prior authorization.

Financial Management
1. Assists the Branch Administrator in managing the operations and clinical services within budget.
2. Responsible for overseeing case management to ensure services that are financially sound.
3. Understands and communicates financial goals and objectives to staff.

Clinical Services, Coordination of Care & Marketing
 
Clinical Services
1. Oversees clinical services provided to patients. Supervises all clinical staff including therapists and therapy aides.
2. Assures documentation of designee in the absence of the supervising nurse.
3. Expected to conduct patient visits and complete clinical activities including but not limited to: assessments, vital signs, oximetry, tube feeding, trach care, suctioning, vent care, infusion or oxygen therapy, etc.
4. Responsible for ensuring availability of resources for all clinical services prior to accepting clients.
5. Ensures provision of training related to clinical practice issues, regulation & reimbursement.
6. Assures staffing of personnel to promote quality services for clients.
7. Ensures provision of training related to clinical practice issues, regulation & reimbursement.
8. Recommends new policies and/or changes in current policies to improve quality of patient services as well as efficiency and effectiveness of services.

Coordination of Care
1. Ensure effective coordination of client care according to the plan of care.
2. Establishes and implements systems to inform patients and employees of the scope and nature of applicable services and community resources.
3. Oversees the scheduling and competency of staff to respond to agency referrals and provide assessment visits within 24‐48 hours of referral.
4. Maintains liaison with attending physicians, uses community resources available for continuity of patient care.
5. Establishes and implements priorities for services within the limitations of available resources.
6. Assures accurate documentation for billing and payroll.
7. Identifies admission criteria for new patients and evaluates eligibility for home health care benefits through third‐party payers. Provides patients and families with information about covered services as well as those services requiring private pay.

Marketing
1. Assists with marketing activities as directed by the Administrator or Regional Director.
2. Provides community awareness and outreach programs for community education.
3. Assists in building relationships and contractual agreements with third‐party payers, other vendors, and the business community.

Compliance & Quality Improvement & Outcomes Management Compliance
1. Applies knowledge of clinical and administrative policies and practices – communicates potential risk management issues and reports abuse or unsafe – illegal practice as required by law.
2. Assures appropriate documentation in clinical record.

4. Ensures the maintenance and confidentiality of patient records and employee files.
5. Is accountable for the implementation of clinical policies in accordance with the Nurse Practice Act.
6. Assures compliance with requirements including but not limited to hiring, documentation & billing aspects and all patient care rules and regs.
7. Ensures compliance with federal/state regulations governing home health care services through policy and procedure administration.

Quality Improvement
1. Assures completion of all activities related to quality improvement processes including but not limited to collecting the following data: outcomes reports, patient satisfaction surveys, clinical record reviews.
2. Evaluates the programs and services annually, identifies opportunities for improvement and develops an action plan. Make recommendations to Administrator and Governing Body.
3. Accountable for acting on QI recommendations and implementing QI plans.
4. Assures completion of quarterly record reviews and evaluates findings and implements action plan for improvement.

Outcomes Management
1. Assists in identifying systems to measure and respond to the outcomes of agency.
2. Assists in developing processes and practices to support quality outcomes as measured by CASPER reports.
3. Assure that agency clinical services meet applicable local, state or federal licensing requirements.
4. Ensure accurate documentation is completed to support medical necessity and reflect physician orders.
5. Ensure quality and safe delivery of home health care services by coordination of care and acts as a resource for all disciplines.
6. Routinely review and evaluate pertinent data related to client services, referrals and discharges to determine effectiveness and propose changes.

Performs other related duties and responsibilities as deemed necessary by the Branch Administrator or Regional Director.

Requirements
1. Sustain Registered Nurse licensure in state of practice.
2. Update and notify of any changes or restrictions to nursing licensure. If restricted, adhere to the guidelines set forth.
3. Obtain and maintain active CPR per agency policy.
4. Manage a case load as needed for client coverage
