  JOB SUMMARY
  
  • Reports to the Manager, Case Management. Serves as liaison by providing support to patients and families in the emergency department through frequent status updates and initiating appropriate contacts in a timely manner. Works with patient and family to provide necessary education and facilitation of linkages with community services and financial resources.     Education and Experience
    
    • The knowledge, skills, and abilities as indicated below are normally acquired through the successful completion of a Master's Degree in Social work. A minimum of 3 years of job related experience preferred. Clinical experience appropriate to designated patient population; management or similar experience in relating across disciplines in a complex health care environment.
