The ACO (Accountable Care Organization) Case Manager is responsible for the assessment, treatment planning, intervention, monitoring, evaluation, and documentation on identified ACO beneficiaries. The ACO Case Manager assesses and develops a care plan in collaboration with the admitting, attending, and consulting physician, the beneficiary and other team practitioners. The goal of the Care Coordination Program is to effectively manage beneficiaries on an outpatient basis to assure the appropriate level of care is provided to prevent in beneficiary re-admissions and ensure that the beneficiary’s medical, environmental, and psychosocial needs are met over the continuum of care.
Education:
Bachelors Degree in Nursing
Experience:
DMAS/DNV/CMS regulations and Conditions Participation knowledge is required
Two years of experience in an acute hospital setting is required
Two years of Case Management is preferred
Licensure:
License to practice in the Common Wealth of VA required
Certification in Case Management or related field preferred
