Peer Support Specialist, Court Based


 


SCOPE OF ROLE


Reporting to the Supervisor, Behavioral Health Services, the Peer Support Specialist, Court Based ultimately serves as a patient advocate. You will be relied on to provide patients with resourceful information and support while providing the Mission Model program to clients of the Ashtabula specialty court dockets.  You will work closely with the courts and Ashtabula MHRS board to provide additional interventions and support to participants.  The Peer Support Specialist, Court Based will model competence to their patients, through their own personal experiences, in successful self-recovery. You will act as a subject matter expert while performing a wide range of tasks to support patients.


 


HOW YOU’LL SUCCEED ____________________________________________________________________



  • After completing the required training during orientation week for Mission Model (web-based training), you will implement the Mission Model curriculum to promote successful outcomes in Ashtabula Mental Health Court and Drug Court.

  • Help to facilitate group sessions and meet with patients individually in their homes and in the community.  Transport patients as needed to support their recovery.

  • Build friendly and positive patient relationships by using knowledge, skills, and training from Certified Peer Specialist training to model, coach, support and advocate with patients.

  • Provide advocacy and outreach on behalf of patients in all situations where patients are unable to advocate for themselves due to symptoms of mental illness.

  • Identify, develop, and access support resources for patients to increase their success in community integration and community inclusion.

  • Create strong relationships and outstanding one-on-one patient care services by assessing patient’s personal accomplishments, strengths, and challenges in order to achieve their stated goals.

  • Partner closely with patients in self-management of critical or crisis situations.

  • Assist with the preparation and recording of the peer support recovery plan and other documents that verify service delivery in a timely manner according to established quality and regulatory standards.

  • Create a weekly schedule that meets existing program productivity standards and supervision requirements.

  • Ensure that all patient visits are documented in the Electronic Health Record (EHR) immediately.  Also, maintain any charting and documentation required by the courts/ MHRS Board.

  • Model your own personal responsibility, self-advocacy, and recovery tools to your patient to help teach and support recovery.

  • Facilitate effective and thorough patient self-review of progress upon each encounter.

  • Represent Signature Health in a professional manner at all times; including maintenance of appropriate professional appearance as outlined in the dress code policy, and demonstration of respectful interactions with patients and team members.

  • Comply with all agency policies and procedures.

  • Maintain all aspects of HIPAA, promoting patient confidentiality and right to privacy.

  • May be required to perform remote services as determined by Signature Health.

  • Adherence and completion of compliance training provided by Signature Health.

  • Responsible for and completes all productivity/documentation requirements.

  • Participate in all assigned staff meetings and staff development programs.

  • Demonstrate appropriate attendance and punctuality in adherence with Signature Health policies.

  • Other duties as assigned.

 


KNOWLEDGE & EXPERIENCE ______________________________________________________________



  • High School Diploma required.

  • Peer Specialist Certification required.

  • Must have personally been in recovery for 2+ years and maintains a recovery program.

  • 2+ years’ experience in a behavioral health work environment preferred.

  • 1+ years of experience working in a court, criminal justice, probation, or related experience is preferred.

  • Must possess a valid unencumbered Ohio driver’s license.

  • American Heart Association (AHA) Basic Life Support (BLS) certification required.

  • Demonstrated knowledge of the local Mental Health System.

  • Must possess basic computer skills to perform job duties including desktop computing, email, time sheet management, electronic health records, using Microsoft Office Applications, ADP and other relevant software.

  • Must have basic electronic communication and internet skills to gather information required for the program or program participants.

WORKING CONDITIONS ___________________________________________________________________



  • Work is normally performed in a typical interior/office/clinical work environment.

  • While hours of operation are generally standard, flexibility to work evenings and extended hours may be required.

  • Requires periods of sitting, standing, telephone, and computer work. 

  • Hearing: adequate to hear clients or patients in person, over the telephone or through telehealth technology.

  • Speaking: adequate to speak to clients or patients in person, over the telephone or through telehealth technology.

  • Vision: Visual acuity adequate to perform job duties, including reading information from printed sources and computer screens.

  • Physical effort required: occasional lifting and carrying items weighing up to 15 pounds, unassisted.

  • Possible exposure to blood borne pathogens while performing job duties.

  • Frequent bending, reaching, and repetitive hand movements, standing, walking, squatting and sitting, with some lifting, pushing and pulling exerted regularly throughout a regular workday.

  • Sufficient dexterity to operate a PC and other office equipment.
