ACT Community Living Support at AuSable Valley Community Mental Health Authority in Oscoda, Michigan

Job Description:

The purpose of a ACT CLS is to provides community living support as a member of the ACT team to assist persons with severe and persistent mental illness. Assists in fostering the highest quality of life for consumers in the community, by providing support and skill building consistent with the IPOS to increase independent functioning and decreasing the need for treatment in more restrictive settings. Services are carried out in accordance with the components and principals of the ACT fidelity model, recovery principles and in keeping with the policies and procedure of the Board. Persons with Lived Experience with mental illness, intellectual/developmental disabilities or substance abuse disorders are encouraged to apply.
