Position Highlights

Work location: Southwestern Health Resources Headquarters, 1603 Lyndon B. Johnson Freeway, Farmers Branch, TX 75234.

Work environment: Care Transformation & Innovation (CTI).

Please note: The Certified Community Health Worker will be an employee of Southwestern Health Resources (SWHR). SWHR is a clinically integrated network formed by Texas Health Resources (THR) and University of Texas Southwestern Medical Center (UTSW).

Work hours:


  • Full-time, 40 hours per week, Day shift.
  • Require 30% travel within the Dallas/Fort Worth Metroplex and surrounding areas, to make home or community visits to engage clients and/or their caregivers or support system, based on department business needs.

Education


  • High School Diploma or equivalent required.
  • Associate's Degree preferred.
Experience

  • Require 1 (one) year Community Health Worker experience.
Licenses and Certifications

  • Require to obtain Certified Community Health Worker certification from Texas Department of State Health Services (DSHS) within 12 months of employment start date.
Skills & Abilities

  • Excellent oral and written communication skills. Exhibits excellent communication skills to the patients, staff, providers, and visitors should be the guiding principles.
  • Proficient in medical terminology and an understanding of chronic diseases, such as diabetes, cardiovascular disease, chronic kidney disease, depression, and hypertension. Able to promote the principles of good basic nutrition and healthy lifestyle choices.
  • Basic computer skills are also necessary for this position.
  • Interpersonal skills - ability to display a caring demeanor towards others.
  • Teamwork - Work collaboratively and effectively within a team environment.
  • Ability to safely drive an automobile Intense.
  • Curiosity - Continuously expand knowledge and understanding of community resources, services and programs provided; human relations and the procedures used in dealing with the public as part of a service or program; volunteer resources and the practices associated with using volunteers, operations, functions, policies and procedures associated with the department or program area, procedures and resources available to handle new, unusual or different situations.

Position Responsibilities

Certified Community Health Workers (CCHWs) are frontline public health workers who have a close understanding of the community they serve. This trusting relationship enables them to serve as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. CCHWs work closely with patients under the direction of a licensed Care Manager and Physician to ensure adherence to a medical plan of care and through an integrated multidisciplinary team approach. Community health workers typically have deep roots or shared life experiences in the communities they serve. They share similar values, ethnic background, and socio-economic status and usually the same language as the people they serve.

Position Functions

  • Help motivate patient to be active, engaged participants in their health through active follow-up in the physician-initiated plan of care and/or the patient-centered action plan developed by a licensed team member. Help patients develop and understand health management plans and goals.
  • Builds the patient's capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support and advocacy. Coach patients in effective management of their chronic health conditions and self-care.
  • Assists patients in referrals, follow-ups, and completing applications for services are done by the community health worker. Establishing a close relationship with the patient is necessary for good comprehensive care. Promote goal setting and strategic planning for the future.
  • Document activities, service plans, and results in an effective manner while strictly adhering to the policies and procedures in place.
  • Help clients in utilizing resources, including scheduling appointments, and assisting with completion of applications for programs for which they may be eligible, coordinate transportation for their patients and conduct eligibility determination of services.
  • Effectively work with people (staff, clients, doctors, agencies, etc.) from diverse backgrounds in reducing cultural and socioeconomic barriers between clients and institutions. Build and maintain positive working relationships with the clients, providers, nurse case managers, agency representatives, supervisors and office staff
