Description:

Assists in the care of acute, chronic or terminally ill patients in their residence under the supervision of a Registered Nurse, Physical Therapist or Occupational Therapist. Provides personal care, rehabilitation services, and certain household services in order to prevent or postpone patient hospitalization or promote palliative care.
Completion of state-approved nurse aide training (CNA) course and listed in good standing or is eligible for re-certification on state nurse aide registry.


Driver's license and auto insurance as required by law.


Must have current BLS, thru American Heart Association, at time of hire.


High school graduate or GED equivalent is required.


Successful completion of competency evaluation program as administered by the department.


One year experience in a hospital, nursing home, home health agency or hospice is preferred.
