Job Summary:
To coordinate the intake and enrollment process for LIFE St. Francis and to promote Life St. Francis to potential participants and community agencies.

What your day will look like:
Your day will largely be spent in the community meeting with prospective participants in their home setting to determine their eligibility for the program. However, there will be timer spent in the office entering your assessments into our electronic charting systems and coordinating with other key staff members in the interdisciplinary team to facilitate further assessments needed for enrollment. You will attend the care plan meeting and deliver a clear and conscious care plan to the participant and family at time of the enrollment signing.

You will be responsible for:
All tasks related to Intake & Enrollment of prospective participants
Growth and retention of organization
Maintaining relationships with referral sources in the community
Communicating and collaborating with Interdisciplinary team and other key staff to ensure a smooth transition for prospective participants
Working with prospective participants and family members to facilitate and submit Medicaid application when needed
Education/Licensure:
Bachelor's Degree preferred (Associates at minimum)
Experience:
2-4 years of related experience preferred
Effective written and verbal communication skills.
Computer efficiency in word processing and spreadsheet applications
Ability to effectively represent PACE philosophy and model to the community and prospective participants
Driver's License
One year working with frail elders
Skills:
Technically inclined and able to work within multiple electronic platforms for documentation and tracking purposes
Strong organization skills
Able to connect and empathize with the elder community
Comfort level with working in the community and entering into prospective participants homes
