Job Description:

GENERAL SUMMARY/ OVERVIEW STATEMENT:


Under the supervision of the Suboxone Nurse Case Manager (NCM) in Adult Medicine at the MGH Health Centers, the Suboxone Medical Assistant II supports the work of the NCM by performing all clerical and supportive clinical duties needed by the program. This includes monitoring program calls and screening potential candidates for the program, and scheduling intake/evaluation visits with the NCM and or physician. CHW also may provide support to the SUDS Initiative by compiling data on the program for reporting purposes.


PRINCIPAL DUTIES AND RESPONSIBILITIES:




  • Answers phones, triage telephone calls and/or take messages for Suboxone Program.

  • Screens potential patients for the Nurse Case Manager (NCM).

  • Schedules intake/evaluations with the NCM.

  • Assists with all computer and telephone issues, computer programs and software.

  • Assists with program material translations from English to Spanish.

  • Functions as a liaison to promote effective working relationships with providers and other agencies and resources.

  • Completes required reports. Faxing reports/information to BMC staff.

  • Complies with the MDPH BSAS Office Based Opioid Treatment Program Policies and Protocols.

  • Collaborates and communicates with the health care team and other involved staff/agencies to ensure appropriate coordination of services, including coordinating supported referrals

  • Collaborates with Mental Health providers, including substance abuse and domestic violence staff, to coordinate appropriate services for patients.

  • Complies with confidentiality policies, HIPAA, JCAHO, and other regulatory requirements.

  • Completes BSAS trainings offered by MDPH and attends MDPH meetings as required.

  • Updates patient registration data and books follow up appointments.

  • Screens potential new patients to the Office Based Addiction Treatment(OBAT) program using EPIC and a screener tool, summarizes the information and then sends on the clinical support staff to further evaluate and decide on next steps.

  • Follows up with patients as needed and provides them with necessary information to facilitate ongoing treatment.

  • Composes correspondence, memoranda and spreadsheets, proofreads documents for typographical errors and makes revisions. Submits documents to supervisor or other designated department personnel for approval.

  • Meets with patients and collects screening information at intake, and quarterly for database reporting.

  • Tracks State data base reporting forms on excel lists and works with the program manager and data coordinator on data reporting.

  • Collects demographic data needed for MDPH/BSAS reports and inputs on to reporting forms.

  • Trains patient, families and other on Narcan administration.

  • Routinely handles large amounts of sensitive, confidential information and maintains confidentiality.

  • Gathers and tracks various patient and non-patient related information, and generates daily and monthly reports as needed for meaningful review.

  • Greets patient in waiting room and escorts to exam room.

  • Assists with admission process: Checks patient's identification using (2) patient identifiers - name and DOB, documents chief complaint or symptom(s) in medical record, as stated by the patient.

  • Instructs patient how to prepare for the nurse (assisting as necessary).

  • Assists the nurse with patient care during the visit.

  • Adheres to hand hygiene and standard precaution guidelines.

  • Collects toxicology specimens during visit and as applicable provides instructions for specimen collection at home:


    • blood (phlebotomy)

    • clean-voided urine

    • oral swabs


  • Documents testing / results per practice protocols.

  • Assists the nurse with work outside the visit related to managing a panel of patients.

  • Performs designated tasks related to tracking test results and may communicate normal test results to patients as directed by the nurse.

  • Assists with detail EPIC lab tracking - closing visits, maintaining tickler files, printing letters for the nurse.

  • Receives patient messages, reviews these with the nurse per practice protocols, and takes action as directed.

  • Assists the nurse in coordinating care with/making referrals to community agencies for non-clinical services.

  • Assures exam and treatment rooms are properly stocked, equipped, and secured as applicable.

  • Assures all equipment is in good working order, cleans equipment after patient use per office protocol.

  • Interacts in a professional, cooperative way with other practice staff.

  • Engages in constructive problem solving with others in the practice as problems arise.

  • Attends and participates in practice/program staff meetings.

  • Other duties as assigned.



WORKING CONDITIONS:


Ability to sit for extended periods of time using a computer and telephone



Qualifications

QUALIFICATIONS:


Minimum of HS diploma, BA/BS preferred. Bilingual/bicultural (Spanish) with a minimum 1 to 2 years of clerical experience. Excellent computer skills particularly in MS Office, Outlook. Good telephone manner. Ability to communicate well with diverse populations.
