Judge Healthcare is currently seeking a Case Manager RN in Virginia!



Role: Case Manager RN
Type: Contract
Schedule: 8:30am to 5:00 pm, Monday to Friday
Location: Up to 75% travel.


Locations include: King George, Middlesex, Westmoreland, Lancaster, Northumberland, Essex King and Queen Richmond County.



Responsibilities:

• Face to face visits in homes.


• Care Coordinator (Case Management Coordinator) is responsible for assessing (in person), planning, implementing, and coordinating all case management activities with members to evaluate the medical needs of the member to facilitate the member's overall wellness.


• Develops a proactive course of action to address issues presented to enhance the short and long-term outcomes as well as opportunities to enhance a member's overall wellness through integration.


• Services strategies, policies and programs are comprised of network management and coverage policies.


• Contractual requirement and ratio to meet regulatory requirements.
  
  

Qualifications & Requirements:

• 1 years Case Management experience


• Bachelor's degree


• RN License

Preferred Qualifications:

• Managed Care Experience


• Care Coordination/ Case Management in an integrated model SNF/NF experience of greater than 1 year


• Virginia resident


• Master's Degree Education Bachelor's degree in Social Work, Psychology ,Nursing or Health/Human services related field
