Discharge Coordinator, Regional

The Regional Discharge Coordinator, under the direction of the Case Manager, coordinates post-acute patient level of care needs (SNF's, Sub Acute, LTACH, Home Health Agencies, DME, etc) to facilitate a safe discharge of hospital patients, including barrier patients, to the appropriate setting, assuring continuity of care. Notifies receiving facility of insurance information as needed. Responds effectively to questions and concerns expressed by patients and other staff and resolves identified problems. Refers complex psychosocial issues to social workers. Performs basic reception functions to ensure smooth department operations. Multi-site DCC support/coverage based on facility needs. Provide training and education to DCCs new hires. Assist in monitoring activities related to DCC functions, i.e., documentation, timely discharge coordination by performing chart audits as requested by facility DCM. Updates and maintains community resources on a regular basis, including but not limited to contracted providers.
Job Responsibilities/Duties
Minimum Education: High School Graduate or Equivalent required.
Minimum Experience: Must demonstrate customer service skills appropriate to the job. Computer literacy and proficiency. Excellent written and verbal communication skills. Ability to establish and maintain effective working relationships within the organization. Ability to multitask and maintain a work pace appropriate to workload. Previous experience in an acute care setting or previous experience as discharge coordinator, patient access representative, discharge planning in other healthcare setting, i.e., SNF, ARU, Home Health, Hospice, LTAC. One (1) year experience as discharge coordinator or discharge planner in an acute care setting preferred.
