St. Paul’s Senior Services is excited to announce an amazing opportunity to be part of San Diego’s St. Paul's PACE program (Program of All-Inclusive Care for the Elderly). This innovative program is for individuals 55 years or older, who are living with chronic illness, or disabilities and need coordinated medical care to continue living as independently as possible in their home and community. The healthcare teams at St. Paul’s PACE provide a hands-on approach coordinating medical, social, and home care services so individuals no longer need to manage their medical care alone.
Job Summary:
Under the supervision of St Paul’s PACE Operations Director, the Behavioral Health Program Manager develops plans, organizes, implements, and monitors the functions of St. Paul’s PACE Behavioral Health (BH) direct care services. This position provides oversight of BH programming and quality of care at all St. Paul’s PACE centers. Responsibilities include but are not limited to: development of behavioral health services program for all St. Paul’s PACE participants; developing systems for referrals, authorizations and oversight of behavioral health utilization and quality of care of contracted providers; collaboration with St. Paul’s PACE Interdisciplinary team on participant BH needs; development of behavioral health care coordination systems; development of behavioral health crisis management techniques; training on BH interventions for PACE staff; supervision of PACE BH staff; participation in the care coordination and care management activities of St. Paul’s PACE; and, assessment, care planning, treatment, teaching and counseling to participant, caregiver or other appropriate representatives.
Qualifications and Requirements:
Education: Master’s Degree in Social Work, LCSW or Doctorate in Psychology
