Judge Healthcare is seeking an Travel Case Manager RN for an opening with one of our top clients in the Iselin, NJ area!


Position Details:

Title: Case Manager RN

Type of Engagement: Contract

Rate: Competitive and Dependent on Experience

Location: Iselin, NJ area- telecommute 40%, Field 60%


Duties:

Minimum of 4 completed comprehensive assessments including plan of care (telephonic or in-person). Staff will be provisioned with appropriate Personal Protective Equipment (PPE) and are required to perform this role with patients and members on site, as this is an essential function of this role.

Coordinate care by contacting providers to secure care services for assigned members

Report completed assignments to assigned Clinical Manager

Collaborate with team members to resolve care issues as appropriate

Qualifications:

1.High School Diploma/GED

2. Active, unrestricted RN license in NJ

3. At least 1 year working in a healthcare industry

4. Ability to travel in assigned region to visit Medicaid members in their homes and/or other settings, including community centers, hospitals, or providers' offices

5. COVID vaccination required


Hours:

8 hours/day, 40 hours/week
