Overview

We're looking for an RN Senior Patient Care Manager to join our team in Murfreesboro, TN.

Full-Time, Full Benefits. In addition, we are offering a $ 10,000 hiring incentive.

Three (3) years of current direct patient care experience in a clinical setting (Acute, Post-Acute, LTC/SNF, Oncology, Wound Care...etc.)

Two (2) years of current RN hospice/home health patient care experience.

One (1) year or more of current RN leadership experience in hopsice/home health. (Management of staff, hire, term, corrective action, evaluation, budget....)

Reports directly to the Executive Director or Administrator. You will be responsible for the day to day operations of your assigned hospice site, administering the clinical aspects of the hospice program and for ensuring the provision or quality of care to patients. You will also be responsible for managing, overseeing and directing the services and performance of assigned team members.

• Supervising, managing performance, and directing the work of specified team members, assigning of admit and evaluation visits, scheduling visits and ensuring staff are completing visits in a timely manner


• Managing the site in the absence of the Executive Director, Administrator.


• Managing and overseeing clinical and administrative and recordkeeping functions


• Responding to phone referral/inquiries in a timely, professional, and compassionate manner


• Participating in on-call nursing schedule and providing direct skilled bedside nursing care in patients homes

• Degree in Nursing. Registered Nurse with ASN or BSN from an accredited school of nursing, one year experience as an RN (Licensed in the state of requested employment)


• Three years' experience in direct patient care. One year management experience. Two or more years' experience in hospice or home care required.


• Must be able to effectively operate nursing assessment tools and office equipment to include, computer and or tablet


• Sensitivity to feeling of the terminally ill and their families before and after a patient's death


• Knowledge of terminally ill patients and their families along with an understanding of the hospice concept
