Job Description:

Nemours is seeking a Behavioral Health Care Coordinator-Psychology in Wilmington, DE.

This is a two-year grant funded position with the possibility of an extension. At the conclusion of the grant, the candidate will be eligible for an internal transfer to a position if available.

Primary Functions:

This position supports patients and families receiving care within Nemours Children's Hospital by providing linkage to community resources, financial assistance, and positive family experiences. Responsibilities include but are not limited to:

Maintaining relationships with community partners including psychiatric care facilities

Collaboration with child welfare partners in DE and neighboring states

Family interviewing and psychosocial assessment

Providing social work support to the behavioral health outpatient clinic including supporting transition to higher level of care needs

Coordination of care with internal partners and departments

Financial resource application completion and tracking

Facilitating, organizing, and tracking in kind community donations to patients/families

Essential Functions:

  • Attend huddles and rounds in accordance with team expectations.
  • Coordinate services/resources in collaboration with the multidisciplinary team.
  • Establish and maintain effective working relationships across disciplines.
  • Document patient care activity in patient medical record according to departmental policy.
  • Keep apprised of community resources and provide resource specific information based on the assessed needs of the patient and family.
  • In conjunction with other team members, establish and maintain productive and effective relationships with families.
  • Complete applications for financial assistance and positive family experiences. Facilitate their fulfillment.
  • Assist with crisis assessment and support outpatient team when families need transport to acute care facilities
  • Additional miscellaneous duties and responsibilities, as may be assigned from time to time by employee's supervisor.

  • Job Requirements:

    Bachelor's degree in Social Work

    Proof of application for Licensed Bachelor's Social Worker (LBSW) within 90 days of employment. Procurement of LBSW within one year of employment.

    Licensed Bachelor's Social Worker preferred.

    1-3 years of job related experience. Internship may be applied to the job-related experience at the discretion of the hiring manager
