Description:

Under the supervision of the PACE Nutrition Manager, and in collaboration with the PACE Registered Dietitians (RDs), the PACE Food Access Coordinator (FAC) ensures PACE participants have adequate access to food through various streams. The FAC’s primary responsibility is to coordinate and complete grocery shopping for Elderly PACE participants who require assistance, both online and in-person. Other responsibilities include facilitating EBNHC food pantry referrals and assisting with online farmers market orders and/or deliveries, identifying other appropriate sources of community food support, as well as other tasks related to internal meal service operations.

Essential Duties and Responsibilities:


  • Coordinates with RDs to identify participants needing grocery shopping or other food access-related assistance

  • Communicates with PACE participants referred for grocery shopping assistance to gather required information to complete shopping (list, preferences, accepted substitutions, payment source, SNAP benefits, etc.)

  • Completes grocery shopping at participants’ preferred local grocery stores

  • Assists with bringing in and putting away groceries for participants who require help

  • Assists participants to place online grocery orders for home delivery

  • Assists participants to check SNAP balance and communicates any issues to RDs and social work department

  • Coordinates with RDs to maintain participant databases and shopping schedules

  • Documents as appropriate in electronic medical record (EMR) system or alternate tracking system

  • Communicates regularly with RDs regarding any issues or concerns voiced by participants

Other Duties:


  • Maintains working knowledge of SNAP, HIP, EBNHC food access efforts (Farmers Market, CSA Program, Food Pantry), educates participants and/or staff as needed

  • Assists participants with referrals to community food pantries including EBNHC food pantry, and maintains records

  • Participates in PACE community food equity project and supports other EBNHC outreach efforts

  • Supports Office of Food Access grant by assisting with cooking and HIP education classes, and helping participants complete necessary forms  

  • Provides access support for online farmers market, coordinates payment with farmers market manager, and maintain records

  • Researches and identifies other sources of community food assistance for participants who could benefit from additional support

  • Coordinates with Social Work participant navigators to assist participants with the SNAP application process

  • Identifies and communicates new food insecurity concerns to RDs and social work department

  • Communicates with participants in person or over the phone for follow-up on food access issues as needed

Qualifications and Requirements:


  • High school diploma or equivalent required; Associates degree a plus

  • Two years’ experience working in a community-based health or human services agency, food access programs a plus

  • Bilingual in Spanish required

  • Valid driver’s license and personal vehicle required

  • Travel and some heavy lifting required

  • Good written and verbal communication skills

  • Good inter-personal and computer skills (proficient in MS Office)

  • Desire to work in a helping profession with people in need

  • Dependable and able to work independently

  • Some flexibility of schedule required with possibility of occasional remote work

  • Will need to obtain ServSafe food safety certification
