Description:

Tired of being evaluated by daily minutes and not by your positive impact on the lives of your clients? Join East Boston Neighborhood Health Center’s  PACE program!

 As part of a dynamic Interdisciplinary Team (IDT) in an innovative model of care,  you will develop plans of care for participants who want to stay safely in their homes and out of skilled nursing facilities.  You will treat participants in their homes and/or on an outpatient basis at one of our PACE centers, participate in developing Wellness services,  and supervise COTAs and rehab aides to help participants achieve their goals.  You must be passionate about serving adults 55+ and have at least one year of experience in geriatric settings.  Bilingual English/Spanish a plus. 

Full-time, 40 hours/week
Schedule: M-F, 8 am - 4:45 p.m., Lunch break: 12-12:45
