100% FTE, core operations occur during traditional business hours and occasional hours may be offered outside of this timeframe. Typical schedule is Monday - Friday, 8:00am - 4:30pm. Hours may vary based on the operational needs of the department. You will be working at University Hospital in Madison, WI.

We are seeking a Registered Nurse (RN) Transitional Care Case Manager to:



  • Help assure care coordination across the care continuum.

  • Bridge inpatient to ambulatory care for Medical patients; primarily Cardiology and Family Medicine Service lines.

  • Coordinate with interdisciplinary teams to ensure remarkable healthcare.

  • Use clinical expertise to empower patients during post-acute hospitalization recovery.

  • Work with a dynamic, innovative and fun interdisciplinary team.

  • Practice at the top of your license.

Education:

Minimum – Bachelor's degree in Nursing (BSN).

Preferred – Master’s degree in Nursing or Health Care related field.

Work Experience:

Minimum – Three (3) years of relevant clinical nursing experience. Equivalent combination of education and experience will be considered.

Preferred – Surgical, trauma, and intensive care experience.

Licenses and Certifications:

Minimum – RN licensed in the state of Wisconsin CPR certification.

Preferred – ACMA certification as a case manager.
