Job Description:

An award-winning Level 1 Trauma Center with 500+ beds in Virginia is looking for an experienced Manager of Case Management. 

This Magnet designated facility is one of the only Level 1 Trauma Center's in Virginia and is part of a system with plenty of room for growth and expansion. This hospital is the primary teaching hospital in the region and provides several specialized services that other hospitals in the region do not. This hospital is known for it's heart and vascular services, neurosurgery, oncology care, advanced imaging, and state of the art NICU.

This hospital is looking for an experienced Case Management leader who will report to the Director of Case Management and oversee a team of twelve Registered Nurses. This position will be responsible for management of the inpatient care transitions program. The Manager will participate in system development and and implementation of integration of care transitions. The Manager will serve as a mentor and provide professional practice consistent with organizational goals, service, effectiveness, and standards of care.

REQUIREMENTS: 

• BSN Required


• MSN (Preferred)


• Specialty certification required within one year of eligibility (ACM, CCM, CCCTM, CMAC
  or CMGT-BC).


• Active Nursing License
